Description
Overview of the medicine
Remacid Plus is a combination medicine containing Esomeprazole, a proton pump inhibitor, and Domperidone, a prokinetic agent. It is used to treat gastroesophageal reflux disease (GERD), dyspepsia, and other conditions associated with excessive stomach acid and impaired gastric motility.
Uses & Indications
Dosage
Adults
Usually one capsule (Esomeprazole 20 mg + Domperidone 10 mg) once daily, 30-60 minutes before breakfast. In severe cases, dosage may be increased to twice daily as directed by a physician.
Elderly
No specific dosage adjustment required, but caution is advised in patients with severe hepatic or renal impairment.
Renal_impairment
No dosage adjustment necessary for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
Take orally, preferably 30-60 minutes before a meal (usually breakfast) for optimal acid suppression. Swallow the capsule whole with water; do not chew or crush.
Mechanism of Action
Esomeprazole works by irreversibly blocking the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion. Domperidone acts as a peripheral dopamine D2 receptor antagonist, enhancing gastrointestinal motility and reducing nausea and vomiting.

Absorption
Esomeprazole: Rapidly absorbed, bioavailability 50-68%. Domperidone: Rapidly absorbed, low absolute bioavailability (15%) due to first-pass metabolism.
Metabolism
Esomeprazole: Hepatic via CYP2C19 and CYP3A4. Domperidone: Hepatic via CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole, Domperidone, or any excipients
- •Patients with prolactin-releasing pituitary tumor (prolactinoma)
- •Patients with moderate to severe hepatic impairment (due to domperidone)
- •Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
- •Conditions where stimulation of gastrointestinal motility is harmful (e.g., gastrointestinal hemorrhage, mechanical obstruction, perforation)

Drugs that depend on gastric pH for absorption (e.g., Ketoconazole, Itraconazole, Erlotinib, Iron salts): Esomeprazole can reduce their absorption.

Clopidogrel: Esomeprazole may reduce the antiplatelet effect of Clopidogrel.

Drugs metabolized by CYP3A4 (e.g., Erythromycin, Ketoconazole, Amiodarone, some antidepressants): Concomitant use with Domperidone may increase the risk of QT prolongation.

Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive measures should be initiated. Activated charcoal may be given. Monitor cardiac function (ECG) due to the risk of QT prolongation with domperidone. There is no specific antidote.
Pregnancy & Lactation
Pregnancy: Use with caution during pregnancy only if clearly needed and the potential benefits outweigh the risks. Lactation: Domperidone is excreted in breast milk and may cause adverse effects in infants. Consult a doctor before use during breastfeeding.
