Description
Overview of the medicine
Renopro 10 mg Tablet contains Finerenone, a non-steroidal mineralocorticoid receptor antagonist (MRA) used to reduce the risk of sustained eGFR decline, end-stage kidney disease, cardiovascular death, non-fatal myocardial infarction, and hospitalization for heart failure in adults with chronic kidney disease (CKD) associated with type 2 diabetes.
Uses & Indications
Dosage
Adults
Initial dose is 10 mg once daily if eGFR is 25-60 mL/min/1.73m² and serum potassium is ≤ 4.8 mmol/L. If eGFR is ≥ 60 mL/min/1.73m² and serum potassium is ≤ 4.8 mmol/L, initial dose is 20 mg once daily. Max dose is 20 mg once daily.
Elderly
No specific dose adjustment is required based on age.
Renal_impairment
No dose adjustment is recommended for mild to moderate renal impairment. For severe renal impairment (eGFR < 25 mL/min/1.73m²), initiation is not recommended.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole; do not crush, chew, or split.
Mechanism of Action
Finerenone selectively blocks overactivation of the mineralocorticoid receptor (MR), which is implicated in the pathophysiology of CKD and heart failure. MR overactivation contributes to inflammation and fibrosis.
Pharmacokinetics
Onset
Within hours for pharmacodynamic effects (e.g., UACR reduction) but clinical benefits are long-term.
Excretion
Approximately 80% eliminated via feces (primarily as metabolites) and 20% via urine (mostly as metabolites).
Half life
Approximately 2-3 hours for the parent compound, 19-24 hours for the active major metabolite.
Absorption
Rapidly absorbed, absolute bioavailability is approximately 44%. Peak plasma concentration is reached within 0.5-1.25 hours.
Metabolism
Primarily metabolized by CYP3A4 (90%) and to a lesser extent by CYP2C8 (10%).

Contraindications
- Concomitant use with strong CYP3A4 inhibitors.
- Adrenal insufficiency.

Symptoms of overdose include hyperkalemia. Management includes close monitoring of potassium levels, supportive care, and consideration of calcium gluconate or insulin with glucose for severe hyperkalemia.

Pregnancy: Limited human data. Animal studies show adverse effects. Use only if potential benefit justifies potential risk to fetus. Lactation: Unknown if excreted in human milk. Advise against breastfeeding during treatment.
