Description
Overview of the medicine
Renopro 20 mg Tablet contains Telmisartan, an Angiotensin II Receptor Blocker (ARB), used primarily to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events in patients at high risk. It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
The usual recommended starting dose is 40 mg once daily. For 20 mg, it may be used for milder cases or as titration. The dose may be increased to a maximum of 80 mg once daily if needed.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment or dialysis, lower starting doses may be considered, and careful monitoring is advised.
How to Take
Renopro tablet should be swallowed whole with water, with or without food, at approximately the same time each day.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This inhibition leads to vasodilation, reduction in aldosterone secretion, and a decrease in blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect starts within 3 hours; maximum effect achieved within 4-8 weeks.
Excretion
Almost exclusively excreted in feces, primarily as unchanged drug. Less than 1% excreted in urine.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 0.5 to 1 hour. Absolute bioavailability is about 42% for a 20 mg dose.
Metabolism
Primarily metabolized by conjugation to form an acylglucuronide; no cytochrome P450 involvement.
Side Effects
Contraindications
- Hypersensitivity to Telmisartan or any component of the formulation.
- Pregnancy (especially second and third trimesters).
- Lactation.
- Biliary obstructive disorders.
- Severe hepatic impairment.
- Concomitant use with aliskiren in patients with diabetes or renal impairment.
Drug Interactions
Digoxin
Increased plasma levels of digoxin.
Lithium
Increased serum lithium concentrations and toxicity.
Ramipril
Increased risk of hypotension, hyperkalemia, and renal impairment when co-administered with ACE inhibitors.
Diuretics and other antihypertensive agents
Increased hypotensive effect.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal function deterioration.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose would be hypotension and tachycardia; bradycardia might occur. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
Pregnancy & Lactation
Renopro is contraindicated during the second and third trimesters of pregnancy due to risks to the fetus. It is not recommended during the first trimester. Due to the potential for adverse effects on the nursing infant, use is not recommended during lactation.
