Description
Overview of the medicine
Reocef 500 mg Capsule contains Cefradine, a first-generation cephalosporin antibiotic, used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Usually 250 mg to 500 mg every 6 hours or 500 mg to 1 g every 12 hours, depending on the severity of infection. Max 4 g/day.
Elderly
Dosage adjustment may be necessary in elderly patients, especially with impaired renal function.
Renal_impairment
Reduced dosage is required. For CrCl 10-50 mL/min, 250 mg every 6-12 hours; for CrCl <10 mL/min, 250 mg every 12-24 hours.
How to Take
Oral administration. Can be taken with or without food. For best absorption, it is often recommended to take it with food to reduce GI upset.
Mechanism of Action
Cefradine is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Onset
Typically within 30-60 minutes.
Excretion
Mainly via the kidneys (glomerular filtration and tubular secretion).
Half life
Approximately 0.7-1.5 hours in adults with normal renal function.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, even in the presence of food.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefradine or other cephalosporins
- History of severe hypersensitivity reactions to penicillin
Drug Interactions
Probenecid
May increase cefradine levels by decreasing renal tubular secretion.
Aminoglycosides
Increased risk of nephrotoxicity with concomitant use.
Oral anticoagulants (e.g., Warfarin)
May enhance anticoagulant effect; monitor INR.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, diarrhea, and possibly neuromuscular hyperexcitability or seizures in severe cases. Management involves supportive care, gastric lavage or induced emesis if acute, and hemodialysis may be beneficial.

Pregnancy category B. Generally considered safe, but use with caution and only if clearly needed. Excreted in breast milk in small amounts, generally considered safe for nursing infants but monitor for diarrhea or candidiasis.
