Description
Overview of the medicine
Repopain is a novel centrally acting analgesic indicated for the management of chronic neuropathic pain and certain types of persistent musculoskeletal pain. It works by modulating specific pain pathways in the central nervous system.
Uses & Indications
Dosage
Adults
Initial dose 100 mg once daily, titrated up to 300 mg twice daily based on response.
Elderly
Start with a lower dose (e.g., 50 mg once daily) and titrate carefully.
Renal_impairment
Dose adjustment required; reduce dose by 50% for moderate impairment, avoid in severe impairment.
How to Take
Take orally with or without food. Extended-release tablets should be swallowed whole, not crushed or chewed.
Mechanism of Action
Repopain selectively modulates voltage-gated ion channels and neurotransmitter release in ascending pain pathways, reducing the transmission of nociceptive signals to the brain. It also enhances descending inhibitory pathways.
Pharmacokinetics
Onset
Onset of action within 1-2 hours.
Excretion
Excreted mainly through urine, small amount via feces.
Half life
Elimination half-life of 6-8 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, bioavailability ~70%.
Metabolism
Primarily metabolized in the liver via CYP3A4 pathway.
Side Effects
Contraindications
- Hypersensitivity to Repopain or any component of the formulation
- Severe hepatic impairment
Drug Interactions
CYP3A4 Inducers (e.g., rifampicin)
Decreased Repopain plasma levels.
CYP3A4 Inhibitors (e.g., ketoconazole)
Increased Repopain plasma levels.
CNS Depressants (e.g., alcohol, benzodiazepines)
Increased sedation and dizziness.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe dizziness, drowsiness, confusion, and possibly seizures. Treatment is supportive and symptomatic; gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Use during pregnancy only if potential benefit outweighs potential risk to the fetus. Not recommended during breastfeeding due to unknown excretion into breast milk.
