Description
Overview of the medicine
Reptylin 25 mg Tablet is an antidepressant belonging to the Selective Serotonin Reuptake Inhibitor (SSRI) class. It is primarily used to treat major depressive disorder, anxiety disorders, and obsessive-compulsive disorder (OCD) by helping to restore the balance of a certain natural substance (serotonin) in the brain.
Uses & Indications
Dosage
Adults
Initial dose is 25 mg once daily. May be increased gradually to 50 mg or up to a maximum of 100 mg daily, based on patient response and tolerability. Doses greater than 50 mg should be divided (e.g., 25 mg twice daily).
Elderly
Start with a lower dose, typically 12.5 mg once daily. Adjust dose cautiously based on individual response and tolerability.
Renal_impairment
No specific dose adjustment is generally required for mild to moderate renal impairment. For severe impairment, caution is advised, and a lower starting dose (e.g., 12.5 mg) may be considered.
How to Take
Take Reptylin 25 mg Tablet orally, with or without food, preferably at the same time each day, usually in the morning or evening. Do not crush, chew, or break the tablet.
Mechanism of Action
Reptylin works by selectively inhibiting the reuptake of serotonin in the brain, thereby increasing the concentration of serotonin in the synaptic cleft. This enhanced serotonergic neurotransmission contributes to its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Initial effects may be noticed within 1-2 weeks, but full therapeutic effect typically takes 2-4 weeks.
Excretion
Excreted predominantly in the urine (approximately 60%) and feces (approximately 30%) as metabolites.
Half life
Approximately 24-36 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, peak plasma concentrations reached within 4-8 hours. Bioavailability is approximately 80%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, CYP2D6, and CYP3A4 enzymes, into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Serotalin or any excipients of the formulation.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- Concurrent use with Pimozide or Thioridazine.
Drug Interactions
MAOIs
Concomitant use is contraindicated due to the risk of Serotonin Syndrome.
Triptans
Increased risk of Serotonin Syndrome.
Warfarin
Increased risk of bleeding due to Reptylin's potential to inhibit CYP2C19, affecting warfarin metabolism.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, drowsiness, tachycardia, tremor, and in severe cases, seizures or serotonin syndrome. Management is primarily supportive, including maintaining an open airway, cardiac monitoring, and symptomatic treatment. Gastric lavage and activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Pregnancy Category C/D. Use only if potential benefit outweighs the risk to the fetus. Exposure during late pregnancy may be associated with persistent pulmonary hypertension of the newborn (PPHN). Reptylin is excreted in human breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
