Overview of the medicine
Resitone 20 mg Tablet contains Esomeprazole, a proton pump inhibitor, used to reduce the amount of acid produced in the stomach. It is commonly prescribed for conditions like gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20 mg or 40 mg once daily for 4-8 weeks. Erosive Esophagitis Maintenance: 20 mg once daily. H. pylori eradication: 20 mg twice daily for 7-10 days (in combination therapy). Zollinger-Ellison Syndrome: 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is generally required for patients with renal impairment.
How to Take
Take the tablet whole with a glass of water, at least one hour before a meal. Do not chew or crush the tablet.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion. This leads to a dose-dependent inhibition of both basal and stimulated acid secretion.

Onset
Within 1 hour, full effect within 2-4 days of continuous use.
Excretion
Approximately 80% is excreted as metabolites in the urine and the remainder in the feces.
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma levels occur within 1-2 hours. Bioavailability is approximately 64% after a single 20 mg dose, increasing to 89% after repeated administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
May increase the absorption and plasma concentration of digoxin.
Diazepam
May decrease the metabolism of diazepam, leading to increased plasma concentrations.
Warfarin
Increased INR and prothrombin time have been reported, requiring monitoring.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting its metabolism.
Methotrexate
May increase and prolong plasma levels of methotrexate and/or its metabolite, leading to potential toxicity.

It is unknown whether Esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
