Description
Overview of the medicine
Paracetamol is a widely used over-the-counter analgesic (pain reliever) and antipyretic (fever reducer). It is commonly used for the relief of fever, headaches, muscle aches, arthritis, backaches, toothaches, colds, and menstrual cramps.
Uses & Indications
Dosage
Adults
1 gm every 4-6 hours as needed. Maximum 4 gm in 24 hours.
Elderly
No specific dosage adjustment usually needed, but caution with liver/kidney impairment.
Renal_impairment
Dosage interval may need to be extended in severe impairment (e.g., creatinine clearance <10 mL/min).

Oral administration. Can be taken with or without food.
Mechanism of Action
Paracetamol is believed to work primarily by inhibiting prostaglandin synthesis in the central nervous system, which helps to reduce pain and fever. Pharmacokinetics
Onset

Excretion
Excreted in the urine primarily as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1.25 to 3 hours in adults, slightly longer in neonates and patients with cirrhosis.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver by conjugation with glucuronic acid and sulfate. A small amount is metabolized by cytochrome P450 enzymes to a highly reactive intermediate (N-acetyl-p-benzoquinone imine, NAPQI), which is detoxified by conjugation with glutathione.
