Overview of the medicine
Revira 500 mg Tablet is a broad-spectrum antibiotic used to treat various bacterial infections, including respiratory tract infections, urinary tract infections, skin and soft tissue infections, and some sexually transmitted infections. It belongs to the fluoroquinolone class of antibiotics.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily for 7-14 days, depending on the infection. For specific infections, duration may vary.
Elderly
No dose adjustment is necessary for elderly patients with normal renal function.
Renal_impairment
Dose adjustment required. For CrCl 20-49 mL/min, initial 500 mg, then 250 mg daily. For CrCl 10-19 mL/min, initial 500 mg, then 250 mg every 2 days.
How to Take
Revira tablets can be taken with or without food. It should be swallowed whole with a sufficient amount of liquid. Do not crush, chew, or break the tablet.
Mechanism of Action
Levofloxacin, the active ingredient in Revira, acts by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA processes and ultimately bacterial cell death, exhibiting a bactericidal effect.

Onset
Within 1-2 hours of administration.
Excretion
Primarily renal, with approximately 87% of the administered dose excreted unchanged in urine within 48 hours.
Half life
Approximately 6-8 hours.
Absorption
Rapid and complete absorption after oral administration; peak plasma concentrations reached within 1-2 hours.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to levofloxacin, other quinolones, or any excipients of the tablet
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- Epilepsy or other seizure disorders
- Children and adolescents below 18 years of age (due to risk of joint damage)
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Corticosteroids
Increased risk of tendon disorders, especially in elderly patients.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May increase the risk of CNS stimulation and convulsive seizures.
Antacids, iron supplements, sucralfate, zinc supplements
May decrease the absorption of levofloxacin. Take Revira at least 2 hours before or 2 hours after these agents.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic treatment and supportive measures should be initiated. Gastric lavage may be performed. Monitor ECG due to the potential for QT prolongation. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin from the body.
Pregnancy & Lactation
Revira (Levofloxacin) is generally not recommended during pregnancy or breastfeeding due to potential risks to the fetus/infant, including cartilage damage. It should only be used if the potential benefit justifies the potential risk to the fetus/infant and under strict medical supervision.
