Description
Overview of the medicine
Rhinocort (Budesonide) is a corticosteroid nasal spray used to treat symptoms of allergic rhinitis, such as sneezing, runny nose, and nasal congestion, and also for non-allergic rhinitis and nasal polyps.
Uses & Indications
Dosage
Adults
Usually 64 mcg (2 sprays) per nostril once daily, or 32 mcg (1 spray) per nostril twice daily. Maximum 256 mcg/day.
Elderly
No dose adjustment typically required.

How to Take
For intranasal use only. Shake gently before use. Clear nasal passages before administering. Prime the pump before first use or after 24 hours of non-use.
Mechanism of Action
Budesonide is a potent glucocorticoid that exhibits strong anti-inflammatory activity. It works by reducing the inflammation in the nasal passages, thereby alleviating symptoms like congestion, sneezing, and runny nose.
Pharmacokinetics
Onset
Onset of action within 10 hours, full effect within 2-7 days.
Excretion
Excreted via urine and feces as metabolites.
Half life
Approximately 2-3 hours.
Absorption
Low systemic absorption (approx. 34% bioavailable orally, even less intranasally due to first-pass metabolism).
Metabolism
Extensive first-pass metabolism in the liver by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to budesonide or any component of the formulation.
- Untreated localized infection involving the nasal mucosa.
Drug Interactions
Cimetidine
Cimetidine and grapefruit juice may also increase budesonide levels.
Ketoconazole
Ketoconazole and other strong CYP3A4 inhibitors (e.g., itraconazole, clarithromycin, ritonavir) may increase systemic exposure to budesonide.
Storage
Store upright at room temperature (20-25°C), protect from light and freezing. Do not refrigerate.
Overdose
Acute overdose is unlikely to cause serious problems. Chronic high doses may lead to systemic corticosteroid effects like Cushing's syndrome or adrenal suppression.
Pregnancy & Lactation
Use in pregnancy only if potential benefit justifies potential risk to the fetus (Category B). Caution advised during lactation as budesonide is excreted in breast milk.
