Overview of the medicine
Richcal-DX is a combination preparation of Calcium Carbonate and Vitamin D3, used to treat and prevent calcium and vitamin D deficiencies, particularly in conditions like osteoporosis and osteomalacia.
Uses & Indications
Dosage
Adults
1-2 tablets daily, or as directed by physician.
Elderly
Same as adult dosage, adjustments may be needed based on kidney function.
Renal_impairment
Use with caution, monitor calcium levels. Reduced dosage or avoidance in severe cases.
How to Take
Oral administration. Can be taken with or without food, but generally recommended to take with food to enhance absorption.
Mechanism of Action
Calcium carbonate provides elemental calcium essential for bone formation, nerve function, and muscle contraction. Vitamin D3 (cholecalciferol) aids in the absorption of calcium from the gut and its deposition into bones, regulating calcium and phosphate homeostasis.

Excretion
Calcium is excreted via kidneys and feces. Vitamin D metabolites are excreted primarily in bile and feces, with some renal excretion.
Half life
Calcium (variable, related to bone turnover). Vitamin D3 (about 24 hours, but stored in fat).
Absorption
Calcium absorption varies (20-30%) and is enhanced by Vitamin D3. Vitamin D3 is absorbed from the small intestine, primarily in the jejunum.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D3 (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D3 (calcitriol), the active form.
Side Effects
Contraindications
- •Hypercalcaemia (high calcium levels in blood)
- •Hypercalciuria (high calcium levels in urine)
- •Severe renal impairment
- •Known hypersensitivity to calcium carbonate or vitamin D3 or any component of the formulation
Drug Interactions
Corticosteroids
May reduce calcium absorption.
Thyroid hormones
Calcium may impair absorption of levothyroxine; separate administration.
Thiazide diuretics
May increase risk of hypercalcaemia.
Tetracycline and fluoroquinolone antibiotics
May reduce absorption of these antibiotics; separate administration by at least 2-3 hours.
Storage
Store in a cool, dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, constipation, thirst, polyuria, anorexia, weakness, and hypercalcaemia. Treatment involves stopping the medication, hydration, and medical management of hypercalcaemia.
Pregnancy & Lactation
Generally considered safe during pregnancy and lactation within recommended doses. Consult a physician before use.
