Description
Overview of the medicine
Rifaximin is a non-systemic antibiotic used to treat traveler's diarrhea caused by non-invasive strains of E. coli in adults and adolescents (12 years of age and older). It works locally in the gastrointestinal tract with minimal systemic absorption.
Uses & Indications
Dosage
Adults
For Traveler's Diarrhea: 200 mg orally three times daily for 3 days. For other indications (IBS-D, HE), higher doses (e.g., 550 mg) are typically used.
Elderly
No dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is required in patients with renal impairment due to minimal systemic absorption.
How to Take
Take orally with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Rifaximin binds to the beta-subunit of bacterial DNA-dependent RNA polymerase, thereby inhibiting bacterial RNA synthesis. This highly localized action primarily occurs within the gastrointestinal lumen.
Pharmacokinetics
Onset
For traveler's diarrhea, symptomatic relief typically begins within 24-48 hours.
Excretion
Mainly excreted unchanged in the feces (>96% of the administered dose). Less than 0.02% is excreted in urine.
Half life
Plasma half-life is approximately 5-6 hours for the minimally absorbed portion.
Absorption
Less than 0.4% is absorbed systemically after oral administration.
Metabolism
Primarily metabolized by CYP3A4, but due to very low systemic absorption, systemic metabolism is minimal.
Side Effects
Contraindications
- •Hypersensitivity to rifaximin, rifamycins, or any component of the formulation.
- •Diarrhea complicated by fever and/or blood in the stool (indicates invasive bacterial pathogens).
Drug Interactions
Warfarin
Although minimal systemic absorption, monitor INR if coadministered, especially in patients with hepatic impairment.
Cyclosporine
Coadministration with P-glycoprotein inhibitors like cyclosporine may significantly increase systemic exposure to rifaximin.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. In case of overdose, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether rifaximin is excreted in human milk; caution should be exercised when administered to a nursing mother.
