Overview of the medicine
Rifaximin is a non-systemic antibiotic used to treat traveler's diarrhea, irritable bowel syndrome with diarrhea (IBS-D), and to reduce the risk of overt hepatic encephalopathy (HE) recurrence.
Uses & Indications
Dosage
Adults
IBS-D: 550 mg orally three times daily for 14 days. Hepatic Encephalopathy: 550 mg orally twice daily. Traveler's Diarrhea (generally 200 mg tid for 3 days, but 550 mg may be used off-label by some practitioners): Refer to specific guidelines.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No dosage adjustment required for patients with renal impairment due to minimal systemic absorption.
How to Take
Take orally with or without food. Complete the full course of treatment as prescribed, even if symptoms improve earlier.
Mechanism of Action
Rifaximin binds to the beta-subunit of bacterial DNA-dependent RNA polymerase, inhibiting bacterial RNA synthesis and protein synthesis, leading to bacterial death. Due to its poor systemic absorption, it acts primarily in the gastrointestinal tract.

Side Effects
Contraindications

Drug Interactions
Warfarin
May alter INR due to changes in gut flora, monitor INR closely and adjust warfarin dose as needed.
P-glycoprotein inhibitors (e.g., ciclosporin)
May significantly increase systemic exposure to rifaximin. Co-administration should be done with caution.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, no specific antidote is available. Management involves symptomatic and supportive care. Due to poor systemic absorption, overdose is unlikely to cause significant systemic toxicity.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution is advised during lactation as it is unknown if rifaximin is excreted in human milk.
