Description
Overview of the medicine
Risperidone is an atypical antipsychotic medication used to treat schizophrenia, acute manic or mixed episodes associated with bipolar I disorder, and irritability associated with autistic disorder.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial dose of 2 mg once daily, may be increased to a target dose of 4-6 mg/day. Bipolar mania: Initial dose of 2-3 mg once daily, target dose 1-6 mg/day.
Elderly
For elderly patients with dementia-related psychosis: Initial dose of 0.25 mg twice daily, with gradual upward titration.
Renal_impairment
In patients with renal impairment (creatinine clearance <30 mL/min): Initial dose of 0.5 mg twice daily, with cautious titration. Max dose 1.5 mg twice daily.
How to Take
Risperdex tablets can be taken with or without food. Swallow the tablet whole with water. Do not crush or chew it.
Mechanism of Action
Risperidone is a potent antagonist of serotonin 5-HT2A and dopamine D2 receptors. It also binds to alpha1-adrenergic and H1-histaminergic receptors. Its antipsychotic effect is primarily mediated through combined antagonism of D2 and 5-HT2A receptors.
Pharmacokinetics
Onset
Clinical improvement may be observed within a few days, but full therapeutic effect may take several weeks.
Excretion
Approximately 70% of the dose is excreted in the urine and 14% in the feces. Renal impairment significantly reduces clearance.
Half life
Risperidone: approximately 3 hours; 9-hydroxyrisperidone (active metabolite): approximately 21 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations of risperidone occur within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 to 9-hydroxyrisperidone (paliperidone), which is equipotent to risperidone. CYP3A4 is a minor metabolic pathway.
Side Effects
Contraindications
- •Known hypersensitivity to risperidone or any component of the formulation.
- •Elderly patients with dementia-related psychosis due to increased risk of mortality.
Drug Interactions
Antihypertensive agents
May increase the risk of orthostatic hypotension.
Dopamine agonists (e.g., levodopa)
Risperidone may antagonize the effects of dopamine agonists.
CNS depressants (e.g., alcohol, benzodiazepines)
May potentiate the sedative effects of risperidone.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine)
May increase risperidone plasma concentrations.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Treatment is symptomatic and supportive, including maintaining an airway, ensuring adequate oxygenation and ventilation, and monitoring cardiac function.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as risperidone and its active metabolite are excreted in human milk.
