Overview of the medicine
Risperdex (Risperidone) is an atypical antipsychotic medication used to treat schizophrenia, bipolar disorder (manic or mixed episodes), and irritability associated with autistic disorder in children and adolescents.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 2 mg/day, target dose 4-6 mg/day, administered once daily or twice daily. Bipolar Mania: Initially 2-3 mg/day, target dose 1-6 mg/day. Autism-associated irritability (children/adolescents): Starting dose 0.25-0.5 mg/day, titrated to target 0.5-3 mg/day.
Elderly
Lower starting doses (e.g., 0.5 mg twice daily), with slow and careful titration.
Renal_impairment
Initial dose 0.5 mg twice daily. Dose may be titrated slowly up to 1-2 mg twice daily.
How to Take
Risperdex tablets can be taken orally with or without food. The tablet should be swallowed whole with water.
Mechanism of Action
Risperidone is a selective monoaminergic antagonist with high affinity for serotonin 5-HT2 and dopamine D2 receptors. It also binds to alpha1-adrenergic and, with lower affinity, to alpha2-adrenergic and H1 histaminergic receptors. It does not have affinity for cholinergic muscarinic receptors.

Onset
Clinical effects may be observed within days, with full therapeutic benefit often taking several weeks.
Excretion
Primarily excreted via urine (approximately 70% of dose) and feces (approximately 14% of dose) as unchanged drug and metabolites.
Half life
Risperidone: approximately 3 hours; 9-hydroxyrisperidone (active metabolite): approximately 21 hours.
Absorption
Well absorbed orally, reaching peak plasma concentrations within 1-2 hours (risperidone) and 3-6 hours (9-hydroxyrisperidone, active metabolite). Bioavailability is approximately 70%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 2D6 (CYP2D6) to 9-hydroxyrisperidone, which has similar pharmacological activity to risperidone.
Side Effects
Contraindications
- •Known hypersensitivity to risperidone or any component of the formulation.
- •Increased mortality in elderly patients with dementia-related psychosis.
Drug Interactions
Antihypertensive agents
May enhance the hypotensive effects due to risperidone's alpha-adrenergic blocking activity.
Levodopa and other dopamine agonists
May antagonize the effects of risperidone.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine)
May increase plasma concentrations of risperidone and its active metabolite. Dose adjustment of risperidone may be necessary.
CYP3A4 inducers (e.g., carbamazepine, rifampicin)
May decrease plasma concentrations of the active antipsychotic fraction. Dose adjustment of risperidone may be necessary.
CNS depressants (e.g., alcohol, opioids, benzodiazepines)
May increase the risk of sedation and other CNS depressant effects.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Management involves symptomatic and supportive treatment, including maintaining an open airway, ensuring adequate oxygenation and ventilation, and monitoring cardiovascular function. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs the potential risk to the fetus. Risperidone is excreted in human breast milk; therefore, patients receiving risperidone should not breastfeed.
