Overview of the medicine
Rispolux 2 mg Tablet contains Risperidone, an atypical antipsychotic medication primarily used to treat schizophrenia, bipolar disorder (manic or mixed episodes), and irritability associated with autistic disorder. It helps restore the balance of certain natural substances in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 2 mg once daily, increase to 4-6 mg daily. Bipolar Mania: Initially 2-3 mg once daily, adjust as needed. Max 16 mg/day.
Elderly
Initially 0.5 mg twice daily, may be increased gradually. Caution due to increased sensitivity.
Renal_impairment
Initially 0.5 mg twice daily, may be increased to 1-2 mg twice daily. Dose adjustment necessary.
How to Take
Rispolux 2 mg Tablet should be taken orally, with or without food. It is usually taken once or twice daily, as directed by the physician. The tablet should be swallowed whole with water.
Mechanism of Action
Risperidone is a selective monoaminergic antagonist with high affinity for serotonin 5-HT2 and dopamine D2 receptors. It also antagonizes alpha1-adrenergic and, to a lesser extent, alpha2-adrenergic and H1-histaminergic receptors. Its antipsychotic activity is believed to be mediated through a combination of dopamine D2 and serotonin 5-HT2 antagonism.

Excretion
Approximately 70% excreted in urine and 14% in feces, mainly as metabolites.
Half life
Mean elimination half-life is about 3 hours for risperidone and 20 hours for the active moiety (risperidone + 9-hydroxyrisperidone).

Well absorbed after oral administration, peak plasma concentrations typically reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 to 9-hydroxyrisperidone (active metabolite), and to a lesser extent by N-dealkylation.

- Patients with severe CNS depression or coma.

Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. In severe cases, coma, seizures, and prolonged QT interval may occur. Treatment is primarily supportive; maintain airway, ensure adequate oxygenation and ventilation, and monitor cardiac function. Gastric lavage and activated charcoal may be considered.
