Overview of the medicine
Rispolux 4 mg Tablet contains Risperidone, an atypical antipsychotic used to treat schizophrenia, bipolar disorder, and irritability associated with autistic disorder.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 2 mg/day, increased to 4-6 mg/day. Bipolar mania: Initially 2-3 mg once daily, adjusted to 1-6 mg/day.
Elderly
Initially 0.5 mg twice daily, may be increased gradually. Lower starting doses and slower titration recommended.
Renal_impairment
Initial dose of 0.5 mg twice daily. Dose escalation should be slower, and the therapeutic dose should be halved.
How to Take
Take orally with or without food. It can be taken as a single dose or divided into two doses per day. Tablets should be swallowed whole with water.
Mechanism of Action
Risperidone is a selective monoaminergic antagonist with high affinity for serotonin 5-HT2 and dopamine D2 receptors. It also binds to alpha1 adrenergic and, with lower affinity, to H1 histaminergic and alpha2 adrenergic receptors. It is an antagonist at these receptors, but has no affinity for cholinergic muscarinic receptors.

Onset
Antipsychotic effects may be observed within a few days to weeks, with full effect achieved over several weeks.
Excretion
Excreted primarily via the kidneys (70%) and feces (14%).
Half life
Risperidone: approximately 3 hours; 9-hydroxyrisperidone (active metabolite): approximately 21 hours.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached within 1-2 hours (risperidone) and 3-6 hours (9-hydroxyrisperidone).
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 to 9-hydroxyrisperidone (paliperidone), an active metabolite. Other pathways include N-dealkylation.

Contraindications
- Hypersensitivity to risperidone, paliperidone, or any component of the formulation.
- Severe cardiac insufficiency, recent myocardial infarction, or uncompensated heart failure.
