Description
Overview of the medicine
Cetirizine is a second-generation antihistamine used to relieve symptoms of various allergic conditions, such as seasonal and perennial allergic rhinitis, and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
Adults and adolescents (12 years and over): 10 mg (10 ml syrup) once daily.
Elderly
Elderly patients with normal renal function may take the usual adult dose. For those with renal impairment, dose adjustment is necessary.
Renal_impairment
Moderate renal impairment (CrCl 30-49 ml/min): 5 mg (5 ml syrup) once daily. Severe renal impairment (CrCl <30 ml/min): 5 mg (5 ml syrup) every other day.
How to Take
For oral administration. Can be taken with or without food. Use a measuring spoon/cup for accurate dosing.
Mechanism of Action
Cetirizine is a selective peripheral H1-receptor antagonist. It inhibits the release of histamine and other inflammatory mediators, thereby reducing allergic reactions.

Onset
Onset of action within 20-60 minutes.
Excretion
Primarily excreted unchanged in urine (about 60%) and feces (about 10%).
Half life
Approximately 8-12 hours in adults; prolonged in children and individuals with renal impairment.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1 hour.
Metabolism
Minimal hepatic metabolism, primarily by oxidation.
Side Effects
Contraindications
- •Hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivatives.
- •Severe renal impairment (CrCl <10 ml/min).
Drug Interactions
Theophylline
A small decrease in cetirizine clearance may occur, but no significant clinical interaction is expected at usual therapeutic doses.
CNS Depressants (e.g., alcohol, sedatives)
Concomitant use may increase CNS depression (drowsiness, impaired performance).
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, agitation, restlessness (especially in children), tachycardia, tremor, and urinary retention. Management is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Human data are limited, but generally reassuring. Should be used only if clearly needed. Cetirizine is excreted in breast milk; caution is advised when administered to a nursing mother.
