Description
Overview of the medicine
Robentyl 10 mg Syrup is an anticholinergic antispasmodic medication primarily used to treat functional bowel disorders like Irritable Bowel Syndrome (IBS). It works by relaxing the muscles of the stomach and intestine, thereby reducing muscle spasms and cramps.
Uses & Indications
Dosage
Adults
Adults: 10-20 mg (5-10 ml) three to four times daily. Maximum dose 80 mg/day.
Elderly
Elderly: Use with caution, starting with lower doses. Individualized dosage based on response and tolerance.
Renal_impairment
Renal impairment: Use with caution. Dosage adjustment may be necessary in severe impairment.
How to Take
Administer orally, preferably 30-60 minutes before meals. Can be taken with food if stomach upset occurs.
Mechanism of Action
Dicyclomine is an anticholinergic (antimuscarinic) agent that blocks the action of acetylcholine at muscarinic receptors on smooth muscles, glands, and the central nervous system. This blockage leads to relaxation of smooth muscles in the gastrointestinal tract, reducing spasms and hypermotility.
Pharmacokinetics
Onset
Typically within 1-2 hours.
Excretion
Mainly through urine (75-80%) and feces (10%).
Half life
Approximately 1.8 hours (terminal half-life).
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are achieved in about 60-90 minutes.
Metabolism
Primarily hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to dicyclomine or any component
- Glaucoma
- Myasthenia gravis
- Obstructive uropathy
- Obstructive gastrointestinal disease (e.g., paralytic ileus, pyloric stenosis)
- Severe ulcerative colitis
- Reflux esophagitis
- Unstable cardiovascular status in acute hemorrhage
- Infants less than 6 months of age
Drug Interactions
Digoxin
May increase serum digoxin levels due to delayed gastric emptying.
Antacids
May decrease dicyclomine absorption; separate administration by at least 2 hours.
Phenothiazines
May decrease phenothiazine absorption and potentiate anticholinergic effects.
Opioid analgesics
Increased risk of severe constipation and paralytic ileus.
Other anticholinergics
Increased risk of anticholinergic side effects (e.g., dry mouth, blurred vision, constipation).
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children. Do not freeze.
Overdose
Symptoms include blurred vision, dilated pupils, dry mouth, difficulty swallowing, dizziness, nausea, vomiting, central nervous system stimulation (e.g., excitement, delirium), and rarely coma. Treatment is symptomatic and supportive; gastric lavage, activated charcoal, and physostigmine (antidote) may be considered under medical supervision.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Dicyclomine is excreted in breast milk; advise against use during lactation due to potential for adverse effects in nursing infants.
