Description
Overview of the medicine
Romfen S is a lozenge containing Flurbiprofen, an NSAID, used for the symptomatic relief of sore throat. It provides local anti-inflammatory and analgesic action.
Uses & Indications
Dosage
Adults
For adults and children 12 years and over: One lozenge dissolved slowly in the mouth every 3 to 6 hours as required. Do not take more than 5 lozenges in 24 hours.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients due to potential increased susceptibility to NSAID side effects.
Renal_impairment
Use with caution; dose reduction may be considered in severe renal impairment.
How to Take
Allow the lozenge to dissolve slowly in the mouth. Do not swallow whole, chew, or crush. Move the lozenge around the mouth occasionally while dissolving.
Mechanism of Action
Flurbiprofen inhibits prostaglandin synthesis by reversibly blocking cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes. This reduces inflammation, pain, and fever.
Pharmacokinetics
Onset
Onset of pain relief usually within 15-30 minutes.
Excretion
Mainly excreted in urine as metabolites and conjugates.
Half life
Approximately 3-6 hours.
Absorption
Rapidly absorbed through the oral mucosa. Peak plasma concentrations are achieved within 30-40 minutes after a lozenge.
Metabolism
Primarily metabolized in the liver via cytochrome P450 enzymes (mainly CYP2C9) to hydroxylated metabolites.
Side Effects
Contraindications
- Known hypersensitivity to Flurbiprofen or any excipients
- History of gastrointestinal bleeding or perforation related to previous NSAID therapy
- Active, or history of recurrent peptic ulcer/hemorrhage
- Severe heart failure, renal failure or hepatic failure
- Third trimester of pregnancy
- Patients with asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
NSAIDs may reduce the natriuretic effect of diuretics and may decrease the antihypertensive effect of ACE inhibitors.
Anticoagulants (e.g., Warfarin)
May increase the effect of anticoagulants, increasing risk of bleeding.
Antiplatelet agents (e.g., Aspirin)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep in original packaging.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, headache, drowsiness, and dizziness. Management is symptomatic and supportive. Gastric lavage and activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy: Contraindicated in the third trimester. Use in first and second trimester only if clearly necessary and potential benefits outweigh risks. Lactation: Small amounts may pass into breast milk; use with caution or consider alternatives.

Clinical Trials
Clinical trials have demonstrated the efficacy of flurbiprofen lozenges in providing symptomatic relief for sore throat, including reduction in pain intensity and difficulty swallowing.
Lab Monitoring
- Not typically required for short-term use of lozenges. For long-term systemic NSAID use, renal function and complete blood count monitoring may be considered.
Doctor Notes
- Advise patients to seek medical attention if sore throat persists or worsens, or if new symptoms like high fever, headache, or nausea appear.
- Consider the patient's medical history for NSAID contraindications.

- If symptoms persist for more than 3 days or worsen, consult a doctor.
- Keep out of reach of children.

Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for a missed one.
