Description
Overview of the medicine
Ropinirole is a dopamine agonist primarily used to treat Parkinson's disease symptoms and Restless Legs Syndrome (RLS). It helps to improve motor function and reduce the urge to move legs.
Uses & Indications
Dosage
Adults
Parkinson's Disease: Initial dose 0.25 mg three times daily, gradually increased weekly based on response and tolerability. Restless Legs Syndrome: Initial dose 0.25 mg once daily, 1-3 hours before bedtime, gradually increased every 2 days.
Elderly
Similar to adult dosage, but titration should be done more cautiously. Monitor for adverse effects.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment (CrCl <30 mL/min), consider slower titration or reduced maximum dose.
How to Take
Ropinirole tablets should be taken orally, with or without food. Taking with food may help reduce nausea. Do not crush, chew, or split the tablet.
Mechanism of Action
Ropinirole acts by stimulating dopamine D2 and D3 receptors in the brain, mimicking the effects of dopamine. This helps to restore the dopamine balance that is deficient in Parkinson's disease and RLS.
Pharmacokinetics
Onset
Within 30-60 minutes for some effects, but full therapeutic effect may take days to weeks with titration.
Excretion
Mainly excreted in urine as metabolites (approximately 60% within 24 hours).
Half life
Approximately 6 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP1A2, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to ropinirole or any component of the formulation.
- Severe hepatic impairment (caution recommended).
Drug Interactions
Estrogens (e.g., oral contraceptives)
May reduce ropinirole clearance, requiring dose adjustment.
CYP1A2 Inhibitors (e.g., Ciprofloxacin, Fluvoxamine)
May increase ropinirole plasma levels, requiring dose adjustment.
Dopamine Antagonists (e.g., Neuroleptics, Metoclopramide)
May reduce the efficacy of ropinirole.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, somnolence, postural hypotension, hallucinations, or psychotic behavior. Treatment is symptomatic and supportive, including maintaining vital signs and discontinuing the drug.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Not recommended during lactation as it may inhibit milk production and be excreted into breast milk.

Lab Monitoring
- No specific routine lab monitoring required. Liver and renal function tests may be considered periodically, especially in patients with pre-existing conditions or on high doses.
Doctor Notes
- Counsel patients on potential for sudden onset of sleep and impulse control disorders.
- Titrate dose slowly to minimize side effects, especially orthostatic hypotension and nausea.
- Monitor for psychiatric symptoms, especially in elderly patients or those with a history of psychosis.
- Avoid abrupt withdrawal; taper dose gradually over a week or more.
Patient Guidelines
- Take Ropinol exactly as prescribed by your doctor.
- Do not stop taking this medicine abruptly without consulting your doctor, as it can worsen symptoms or cause withdrawal effects.
- Be aware of potential side effects such as dizziness or drowsiness, especially when starting treatment or increasing dose.
- Report any unusual urges or behaviors to your doctor immediately.
Missed Dose Advice
If a dose is missed, skip the missed dose and take the next dose at the regular scheduled time. Do not double the dose to make up for a missed one.
