Description
Overview of the medicine
Roxiclav is a broad-spectrum antibiotic containing amoxicillin, a penicillin-type antibiotic, and clavulanic acid, a beta-lactamase inhibitor. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Standard dosage: 500 mg/125 mg tablet every 8-12 hours OR 875 mg/125 mg tablet every 12 hours, for 7-10 days depending on infection severity. Higher doses may be required for severe infections.
Elderly
Similar to adults, but dose adjustment may be necessary in case of impaired renal function.
Renal_impairment
Dose adjustments are required based on creatinine clearance. For CrCl 10-30 mL/min: 500 mg/125 mg every 12 hours. For CrCl <10 mL/min: 500 mg/125 mg every 24 hours. Hemodialysis patients: 500 mg/125 mg every 24 hours with an additional dose during and at the end of dialysis.
How to Take
Oral tablets/suspension should be taken at the start of a meal to minimize potential gastrointestinal intolerance and to maximize absorption of clavulanic acid. Do not crush or chew extended-release tablets.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins. Clavulanic acid irreversibly inhibits a wide range of beta-lactamase enzymes commonly produced by bacteria, thereby preventing the degradation of amoxicillin and extending its antibacterial spectrum.
Pharmacokinetics
Onset
Antibacterial effect typically begins within 1 hour.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion. A small amount is excreted in feces.
Half life
Amoxicillin: Approximately 1-1.5 hours; Clavulanic acid: Approximately 1 hour.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration, with peak plasma concentrations occurring after approximately 1-2 hours. Absorption is not significantly affected by food.
Metabolism
Amoxicillin is minimally metabolized; clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- •History of severe hypersensitivity reaction (e.g., anaphylaxis) to any penicillin
- •History of cholestatic jaundice/hepatic dysfunction associated with amoxicillin/clavulanate therapy
Drug Interactions
Warfarin
May prolong prothrombin time; monitor INR closely.
Probenecid
Decreases renal tubular secretion of amoxicillin, increasing its blood levels.
Allopurinol
May increase the incidence of skin rash.
Methotrexate
May increase methotrexate toxicity; monitor for adverse effects.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days.
Overdose
Symptoms of overdose include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. Management is supportive and symptomatic. Hemodialysis may be used to remove amoxicillin/clavulanate from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use during pregnancy should only be considered if the potential benefit outweighs the potential risk. Both amoxicillin and clavulanic acid are excreted into breast milk; use with caution in breastfeeding mothers.
