Overview of the medicine
Roxiquin is a macrolide antibiotic used to treat various bacterial infections including respiratory tract infections, skin and soft tissue infections, and certain genitourinary infections.
Uses & Indications
Dosage
Adults
For adults, the usual dosage is 150 mg twice daily or 300 mg once daily, taken before meals. Treatment duration typically ranges from 5 to 10 days, depending on the infection.
Elderly
No specific dose adjustment is generally required for elderly patients, unless there is severe renal or hepatic impairment.
Renal_impairment
No dose adjustment is needed for mild to moderate renal impairment. Caution is advised in severe renal impairment; a reduced dose may be considered.
How to Take
Roxiquin tablets should be taken orally, preferably before meals (at least 15 minutes before food) with a glass of water, to optimize absorption.
Mechanism of Action
Roxithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thus preventing the growth and multiplication of susceptible bacteria.

Onset
Peak plasma concentrations are usually reached within 1-2 hours after administration.
Excretion
Primarily excreted via bile and feces (about 50%), with a small amount (less than 10%) excreted in urine.
Half life
The elimination half-life is approximately 10-12 hours.
Absorption
Well absorbed orally, but absorption may be decreased by food. Bioavailability is approximately 50%.

- Hypersensitivity to roxithromycin, other macrolides, or any excipients of the product.
- Co-administration with ergot alkaloids (e.g., ergotamine, dihydroergotamine) due to risk of severe vasoconstriction.
- Co-administration with cisapride, pimozide, or terfenadine.
- Severe hepatic impairment.

Digoxin
May increase plasma concentrations of digoxin, necessitating monitoring of digoxin levels.
Warfarin
May potentiate the anticoagulant effect of warfarin, requiring INR monitoring and dose adjustment.
Midazolam
May enhance the effects of midazolam and other benzodiazepines.
Theophylline
May increase theophylline levels, requiring therapeutic drug monitoring.
