Overview of the medicine
Rubitor (Rosuvastatin) is a statin medication used to lower high cholesterol and triglycerides, and to prevent cardiovascular diseases such as heart attack and stroke.
Uses & Indications
Dosage
Adults
Usually 5-20 mg orally once daily. Maximum 40 mg daily.
Elderly
No specific dose adjustment is generally required, but consider starting at 5 mg for patients >70 years.
Renal_impairment
Mild to moderate: no dose adjustment. Severe (CrCl <30 mL/min, not on hemodialysis): start with 5 mg once daily, maximum 10 mg once daily.
How to Take
Take orally once daily, with or without food, at any time of day.
Mechanism of Action
Rosuvastatin selectively and competitively inhibits HMG-CoA reductase, an enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. This leads to a reduction in hepatic cholesterol synthesis and an increase in LDL receptor expression on the liver cell surface, thereby increasing LDL-C uptake and catabolism.
Pharmacokinetics
Onset
Lipid-lowering effects typically begin within 1 week, with maximum effect by 2-4 weeks.
Excretion
Approximately 90% is excreted unchanged in feces, with the remaining 10% excreted in urine.
Half life
Elimination half-life is approximately 19 hours.
Absorption
Peak plasma concentrations are reached approximately 3 to 5 hours after oral administration. Absolute bioavailability is approximately 20%.
Metabolism
Limited metabolism, primarily by CYP2C9. Approximately 10% is metabolized to N-desmethyl rosuvastatin, which has about half the HMG-CoA reductase inhibitory activity.

Contraindications
- Hypersensitivity to rosuvastatin or any component of the formulation
- Active liver disease, including unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Severe renal impairment (CrCl <30 mL/min) with concomitant ciclosporin
Drug Interactions
Warfarin
May potentiate anticoagulant effect, requiring INR monitoring.
Ciclosporin
Concomitant use significantly increases rosuvastatin exposure; avoid or use extreme caution.
Gemfibrozil
Increases rosuvastatin exposure; co-administration is not recommended.
Erythromycin
Decreases rosuvastatin exposure.
Oral Contraceptives
May increase plasma concentrations of ethinyl estradiol and norgestrel.
