Description
Overview of the medicine
Rupatadine is a non-sedating, long-acting, selective peripheral H1-receptor antagonist with additional platelet-activating factor (PAF) antagonistic activity. It is used for the symptomatic treatment of allergic rhinitis and urticaria in children aged 2 to 11 years.
Uses & Indications
Dosage
Adults
For children aged 2 to <12 years: Body weight 10 kg to <25 kg: 5 mg (5 ml) once daily. Body weight ≥ 25 kg: 10 mg (10 ml) once daily. This oral solution is not typically indicated for adults, who usually take 10 mg tablets.
Elderly
No specific dose adjustment for elderly patients with normal renal and hepatic function. Use with caution in those with impaired function.
Renal_impairment
No dose adjustment is required in patients with mild to moderate renal impairment. Caution is advised in patients with severe renal impairment.
How to Take
Administer orally, once daily. Can be taken with or without food. Use the provided measuring device for accurate dosing.
Mechanism of Action
Rupatadine acts as a potent, long-acting, selective peripheral H1-receptor antagonist. It also has antagonistic activity against platelet-activating factor (PAF), a key mediator in inflammatory responses and allergic reactions.
Pharmacokinetics
Onset
Onset of action typically occurs within 1 hour.
Excretion
Excretion primarily occurs via feces (approx. 60.9%) and urine (approx. 34.6%) as metabolites.
Half life
The mean elimination half-life is approximately 5.9 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations (Cmax) achieved approximately 0.5 to 1 hour post-dose.
Metabolism
Extensively metabolized, primarily by cytochrome P450 (CYP3A4) isoenzymes, to active and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to rupatadine or any excipients
- •Children under 2 years of age
Drug Interactions
Grapefruit Juice
Can increase plasma levels of rupatadine due to CYP3A4 inhibition; avoid concomitant use.
Ketoconazole, Erythromycin
These are strong CYP3A4 inhibitors and can increase rupatadine exposure, leading to increased risk of side effects. Co-administration should be avoided or used with caution.
Storage
Store below 30°C. Do not freeze. Keep the bottle tightly closed in the original carton to protect from light. Keep out of reach of children.
Overdose
In the event of an accidental intake of very high doses, symptomatic treatment together with supportive measures should be given. In case of overdose, contact a poison control center or emergency room immediately.
Pregnancy & Lactation
There are no adequate data from the use of rupatadine in pregnant women. Animal studies do not indicate direct or indirect harmful effects with respect to pregnancy, embryonic/fetal development, parturition, or postnatal development. As a precautionary measure, it is preferable to avoid the use of rupatadine during pregnancy. It is unknown whether rupatadine is excreted into human milk. A decision must be made whether to discontinue breastfeeding or to discontinue/abstain from rupatadine therapy taking into account the benefit of breastfeeding for the child and the benefit of therapy for the woman.
