Description
Overview of the medicine
Rupatadine is a non-sedating, long-acting H1-antihistamine used for the symptomatic treatment of seasonal and perennial allergic rhinitis and urticaria.
Uses & Indications
Dosage
Adults
10 mg once daily (e.g., 10 ml of a 1 mg/ml solution or two 5 mg doses).
Elderly
No specific dosage adjustment is usually required, but use with caution due to potential age-related organ function decline.
Renal_impairment
Use with caution. Dosage adjustment may be necessary in severe renal impairment. Consult a physician.
How to Take
Administer orally. Can be taken with or without food. Use the provided measuring device for accurate dosing.
Mechanism of Action
Rupatadine acts as a selective peripheral H1-receptor antagonist. It also demonstrates antagonist activity at platelet-activating factor (PAF) receptors at higher concentrations, contributing to its anti-allergic effects.
Pharmacokinetics
Onset
Within 1 hour.
Excretion
Eliminated primarily via feces (approximately 60%) and urine (approximately 40%) as metabolites.
Half life
Approximately 5.9 hours (for the parent compound, prolonged for active metabolites).
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations (Cmax) are reached approximately 0.75-1.5 hours after intake.
Metabolism
Extensively metabolized, primarily by cytochrome P450 (CYP3A4) isoenzymes, undergoing significant first-pass metabolism.
Side Effects
Contraindications
- Hypersensitivity to rupatadine or to any of the excipients.
- Not recommended for children under 2 years of age.
Drug Interactions
Grapefruit juice
Avoid consumption of grapefruit juice, as it can increase systemic exposure to rupatadine.
CNS depressants/Alcohol
Concomitant use with CNS depressants or alcohol may potentiate sedative effects and impair psychomotor performance.
Ketoconazole/Erythromycin
Coadministration significantly increases systemic exposure to rupatadine.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for rupatadine.
Pregnancy & Lactation
Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus or infant. Limited data are available. Consult a physician.
