Description
Overview of the medicine
Rupatadine is a second-generation antihistamine with selective peripheral H1-receptor antagonist activity. It is used for the symptomatic treatment of allergic rhinitis and urticaria.
Uses & Indications
Dosage
Adults
10 mg once daily
Elderly
No special dose adjustment is required in elderly patients.
Renal_impairment
Caution is advised in patients with renal impairment (creatinine clearance <10 mL/min).
How to Take
Oral administration. Can be taken with or without food.
Mechanism of Action
Rupatadine acts as a long-acting, selective peripheral H1-receptor antagonist. It also has an inhibitory effect on platelet-activating factor (PAF) and mast cell degranulation, contributing to its anti-allergic properties.

Mainly via feces (60.9%) and urine (34.6%) as metabolites.

Absorption
Rapidly absorbed after oral administration, Tmax approximately 0.5-1 hour. Food increases systemic exposure (AUC and Cmax) by about 23% and 26% respectively, without significant clinical relevance.
Metabolism
Extensively metabolized by the liver via CYP3A4 into active and inactive metabolites (e.g., desloratadine, 3-hydroxydesloratadine).

Contraindications

Drug Interactions
Grapefruit juice
Avoid coadministration with grapefruit juice, as it may increase rupatadine levels.
Ketoconazole, Erythromycin, Diltiazem
Coadministration with strong CYP3A4 inhibitors like ketoconazole or erythromycin may increase systemic exposure of rupatadine.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, dry mouth. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy: Limited data. Use only if clearly needed and potential benefit outweighs potential risk to the fetus. Lactation: Unknown if excreted in human milk. Exercise caution.
