Overview of the medicine
Ryth 150 mg Tablet is an antiarrhythmic medication used to restore normal heart rhythm and maintain a regular heartbeat in patients with certain types of heart rhythm disorders (arrhythmias). It works by blocking certain electrical signals in the heart.
Uses & Indications
Dosage
Adults
Initially 150 mg orally every 8 hours. Dosage may be increased to 225 mg every 8 hours after 3-4 days if needed and tolerated. Maximum 300 mg every 8 hours.
Elderly
Lower initial doses (e.g., 100 mg every 8 hours) are recommended, with careful titration due to reduced metabolism.
Renal_impairment
No significant dosage adjustment is usually necessary for mild to moderate renal impairment, but caution is advised. For severe impairment, dose reduction may be considered.
How to Take
Take the tablet whole with water, with or without food. Do not crush or chew. It should be taken at regular intervals as prescribed by the doctor.
Mechanism of Action
Rhythmoxil Hydrochloride is a Class Ic antiarrhythmic agent. It acts by blocking the fast inward sodium current in the cardiac muscle cells, thereby slowing the conduction of electrical impulses in the heart. This action prolongs the refractory period and helps to stabilize the heart rhythm.

Excretion
Excreted in urine (approx. 38%) and feces (approx. 19%) as metabolites and unchanged drug.
Half life
Approximately 2-10 hours (for extensive metabolizers), 10-32 hours (for poor metabolizers).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Bioavailability is about 10-50% due to significant first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP3A4, into two active metabolites (5-hydroxyrhythmoxil and N-depropylrhythmoxil).

Contraindications
- Known hypersensitivity to Rhythmoxil Hydrochloride or any excipients.
- Significant structural heart disease (e.g., uncontrolled heart failure, cardiogenic shock, severe bradycardia, sick sinus syndrome, second- or third-degree AV block without a pacemaker).
Drug Interactions
Digoxin
Increased plasma levels of digoxin, monitor digoxin levels.
Warfarin
Increased anticoagulant effect, monitor INR closely.
Rifampicin
Decreased plasma levels of Rhythmoxil Hydrochloride due to enzyme induction.
Cimetidine, Quinidine
Increased plasma levels of Rhythmoxil Hydrochloride due to inhibition of its metabolism.
Beta-blockers (e.g., Propranolol)
Increased plasma levels of both drugs, leading to enhanced cardiac depression.

Store at room temperature (below 30°C) in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe hypotension, severe bradycardia, asystole, conduction disturbances, and severe ventricular arrhythmias. Treatment is supportive, including vasopressors, intravenous fluids, and external pacing if necessary. Hemodialysis is not effective.
Pregnancy & Lactation
Rhythmoxil Hydrochloride should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, caution should be exercised when administered to a nursing mother. Consult a doctor.
