Description
Overview of the medicine
Ryth 50 mg Suspension is a macrolide antibiotic containing Erythromycin. It is used to treat a wide range of bacterial infections, especially in patients who are allergic to penicillin. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For children (primarily intended use for 50mg/5ml suspension): 30-50 mg/kg/day in divided doses, usually every 6-8 hours. Maximum 1-2 g/day depending on infection severity. Duration usually 7-14 days. For specific infections, refer to clinical guidelines or physician's advice.
Elderly
No specific dose adjustment for elderly with normal renal and hepatic function, but caution with hepatic impairment. Consult physician for appropriate dosage if necessary.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment (CrCl < 10 mL/min), consider reducing dosage or increasing dosing interval. Consult physician.
How to Take
Shake the bottle well before each use. Administer orally, preferably on an empty stomach 1 hour before or 2 hours after meals for better absorption. If gastrointestinal upset occurs, it can be taken with food, although absorption may be reduced. Use a measuring spoon or oral syringe for accurate dosing.
Mechanism of Action
Erythromycin binds to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis. This action is primarily bacteriostatic, but can be bactericidal at higher concentrations or against highly susceptible organisms.
Pharmacokinetics
Onset
Typically within 1-2 hours
Excretion
Mainly via bile and feces; small amount excreted unchanged in urine.
Half life
Approximately 1.5-2 hours
Absorption
Well absorbed orally, but absorption is affected by food. Acid-labile, often enteric-coated or esterified to improve bioavailability.
Metabolism
Primarily hepatic via cytochrome P450 3A4 (CYP3A4) system, forming inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to erythromycin or any macrolide antibiotic
- Pre-existing hepatic dysfunction
- Concomitant use with terfenadine, astemizole, cisapride, pimozide, or colchicine (due to risk of cardiac arrhythmias)
Drug Interactions
Digoxin
Increased digoxin levels, leading to toxicity.
Warfarin
Enhanced anticoagulant effect, leading to increased bleeding risk.
Theophylline
Increased serum concentration of theophylline, leading to toxicity.
Oral Contraceptives
May reduce efficacy of oral contraceptives (rare).
Statins (e.g., simvastatin)
Increased risk of myopathy/rhabdomyolysis due to CYP3A4 inhibition.
Carbamazepine, Valproic Acid
Increased plasma levels of anticonvulsants, leading to toxicity.
QT-prolonging drugs (e.g., antiarrhythmics)
Increased risk of QT prolongation and ventricular arrhythmias.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after 7-14 days (check specific product leaflet). Do not freeze.
Overdose
Symptoms may include nausea, vomiting, diarrhea, and reversible hearing loss (especially with high doses). Management involves symptomatic and supportive care. Gastric lavage or activated charcoal may be considered if recent ingestion. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe for use during pregnancy, but use only if clearly needed. Erythromycin is excreted into breast milk. Use with caution in nursing mothers, monitoring infants for GI upset or thrush.

WHO Essential Medicine
YesClinical Trials
Erythromycin has been extensively studied since its discovery. Numerous clinical trials have established its efficacy and safety in various bacterial infections, particularly in penicillin-allergic patients and pediatric populations. Current research focuses on resistance patterns and new formulations.
Lab Monitoring
- Liver function tests (ALT, AST, bilirubin) in patients with pre-existing hepatic impairment or prolonged therapy.
- Renal function for severe renal impairment.
- PT/INR if co-administered with warfarin.
Doctor Notes
- Counsel parents/guardians on proper reconstitution and administration of suspension.
- Advise on completing the full course to prevent resistance.
- Monitor for signs of cholestatic hepatitis, especially in adults or with prolonged use.
Patient Guidelines
- Complete the full course of antibiotics as prescribed, even if symptoms improve.
- Do not save medicine for future infections.
- Report any severe stomach pain, persistent diarrhea, or yellowing of eyes/skin to your doctor immediately.
Missed Dose Advice
Take the missed dose as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Erythromycin generally does not impair ability to drive or operate machinery. However, if you experience dizziness or visual disturbances, avoid such activities.
Lifestyle Advice
- Maintain good hydration by drinking plenty of fluids.
- Avoid alcohol during treatment as it may exacerbate gastrointestinal side effects.
- Follow a balanced diet.
