Description
Overview of the medicine
Salbutamol syrup is a short-acting beta-2 adrenergic agonist used to relieve bronchospasm in conditions like asthma and chronic obstructive pulmonary disease (COPD). It works by relaxing the smooth muscles of the airways, making it easier to breathe.
Uses & Indications
Dosage
Adults
Oral: 2-4 mg (5-10 ml syrup) three or four times daily. Maximum single dose 8 mg (20 ml).
Elderly
A reduced initial dose may be appropriate for elderly patients. Usually 2 mg (5 ml syrup) three or four times daily.
Children
2-6 years: 1-2 mg (2.5-5 ml syrup) three or four times daily. 6-12 years: 2 mg (5 ml syrup) three or four times daily. Over 12 years: 2-4 mg (5-10 ml syrup) three or four times daily.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe impairment. Monitor for increased side effects.
Hepatic_impairment
No specific dose adjustment recommended.
How to Take
Salbu 2 mg syrup is for oral administration. It can be taken with or without food. Use a measuring spoon or oral syringe to ensure accurate dosing.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors found in the smooth muscle of the bronchi. This stimulation leads to an increase in intracellular cyclic AMP (cAMP) levels, which in turn causes relaxation of bronchial smooth muscle and bronchodilation.
Pharmacokinetics
Onset
Bronchodilator effect typically starts within 5-15 minutes after oral administration.
Excretion
Mainly excreted in the urine as metabolites and unchanged drug within 72 hours.
Half life
The plasma half-life of oral salbutamol is approximately 4-6 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Undergoes extensive first-pass metabolism in the liver, primarily via sulfate conjugation.
Side Effects
Contraindications
- •Hypersensitivity to salbutamol or any component of the syrup
- •Threatened abortion (oral salbutamol can delay labor)
Drug Interactions
Digoxin
May increase digoxin levels, monitor levels if used concomitantly.
Diuretics
Increased risk of hypokalemia, especially with high doses of salbutamol.
Beta-blockers
May antagonize the effects of salbutamol, leading to bronchospasm. Concomitant use is generally contraindicated.
MAOIs and TCAs
May potentiate the cardiovascular effects of salbutamol. Use with caution.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep the bottle tightly closed.
Overdose
Symptoms of overdose include tachycardia, tremors, headache, hyperactivity, hypokalemia, and metabolic acidosis. Treatment is supportive and symptomatic. Consider activated charcoal or gastric lavage in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Salbutamol is excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
