Description
Overview of the medicine
Salix 4 mg Tablet is a potent diuretic used to treat conditions characterized by fluid retention (edema), such as congestive heart failure, liver cirrhosis, and kidney disease. It also helps manage high blood pressure.
Uses & Indications
Dosage
Adults
Initial dose 4 mg once daily. May be increased up to 20 mg per day in divided doses depending on patient response. For hypertension, 4 mg once daily, adjusted based on blood pressure response.
Elderly
Lower initial doses (e.g., 2 mg) and careful titration are recommended due to potential for greater sensitivity and increased risk of electrolyte imbalance.
Renal_impairment
Dosage adjustment required. Lower doses and careful monitoring of renal function and electrolytes are necessary. Contraindicated in anuria.
How to Take
Take Salix 4 mg Tablet orally, with or without food, preferably in the morning to avoid nocturnal diuresis. Follow your doctor's instructions carefully.
Mechanism of Action
Salixum, a loop diuretic, acts by inhibiting the Na-K-2Cl cotransporter in the thick ascending limb of the loop of Henle in the kidney. This prevents the reabsorption of sodium, potassium, and chloride ions, leading to increased excretion of water, sodium, chloride, and other electrolytes.
Pharmacokinetics
Onset
Oral: 30-60 minutes; IV: 5 minutes.
Excretion
Mainly via the kidneys (urine), with some fecal excretion.
Half life
Approximately 0.5-2 hours, prolonged in renal impairment.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability varies but is generally good.
Metabolism
Primarily hepatic, with some renal metabolism.
Side Effects
Contraindications
- Hypersensitivity to Salixum or sulfonamide derivatives
- Anuria (absence of urine production)
- Severe electrolyte depletion (e.g., severe hypokalemia, hyponatremia)
- Hepatic coma and precoma
Drug Interactions
Lithium
Reduced renal clearance of lithium, leading to increased lithium levels and toxicity.
Corticosteroids
Increased risk of hypokalemia.
Aminoglycoside antibiotics
Increased risk of ototoxicity (ear damage).
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the diuretic and antihypertensive effects of Salixum.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dehydration, electrolyte depletion (e.g., hypokalemia, hypochloremia, hyponatremia), hypotension, and possibly renal failure. Treatment is symptomatic and supportive, focusing on fluid and electrolyte replacement.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the risks. Salixum passes into breast milk; therefore, caution is advised during lactation. Consult your doctor.
