Description
Overview of the medicine
Salomax B is a combination of a long-acting beta2-adrenoceptor agonist (salmeterol) and an inhaled corticosteroid (fluticasone propionate). It is used for the regular treatment of asthma and chronic obstructive pulmonary disease (COPD). It helps to open the airways and reduce inflammation.
Uses & Indications
Dosage
0
Adults and adolescents (12 years and older): One inhalation twice daily.
1
Elderly: No dosage adjustment is generally required.
2
Renal impairment: No dosage adjustment is generally required. Hepatic impairment: Use with caution, monitor for systemic corticosteroid effects.
How to Take
For oral inhalation only. Rinse mouth with water after each dose to minimize the risk of oral thrush.
Mechanism of Action
Salmeterol works as a long-acting beta2-agonist, relaxing the smooth muscles in the airways, leading to bronchodilation. Fluticasone propionate is a potent corticosteroid that reduces inflammation in the lungs.
Pharmacokinetics
Onset
Salmeterol: 10-20 minutes. Fluticasone: Onset of full therapeutic effect is gradual, taking several days to weeks.
Excretion
Primarily excreted in feces, with a small portion excreted in urine.
Half life
Salmeterol: Approximately 5.5 hours. Fluticasone: Approximately 8 hours.
Absorption
Salmeterol and Fluticasone Propionate are primarily absorbed via the lungs following inhalation. Systemic absorption of fluticasone is low due to high first-pass metabolism.
Metabolism
Both salmeterol and fluticasone propionate are extensively metabolized by the cytochrome P450 3A4 (CYP3A4) isoenzyme.
Side Effects
Contraindications
- Hypersensitivity to salmeterol, fluticasone propionate, or any excipients.
- Not for relief of acute asthma symptoms or acute exacerbations of COPD (use a rapid-acting inhaled bronchodilator for this).
Drug Interactions
0
Potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir): May increase systemic exposure to salmeterol and fluticasone, leading to increased risk of systemic corticosteroid effects and cardiovascular adverse effects.
1
Beta-blockers: Can block the pulmonary effect of salmeterol and produce severe bronchospasm in patients with asthma. Use with caution.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of salmeterol overdose may include tremor, headache, tachycardia, and hypokalemia. Fluticasone overdose may lead to adrenal suppression. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is not known whether Salmeterol or Fluticasone is excreted in human milk; use with caution in nursing mothers.

Lab Monitoring
- Regular monitoring of lung function (FEV1) and peak expiratory flow. No specific routine lab monitoring required for most patients.
Doctor Notes
- Emphasize proper inhaler technique to patients to ensure optimal drug delivery and minimize local side effects.
- Educate patients that Salomax B is for maintenance therapy and not for acute relief; ensure they have a separate rescue inhaler.
- Monitor for signs of systemic corticosteroid effects, especially in patients on high doses or those with risk factors.
Patient Guidelines
- Use regularly as prescribed; it is not for sudden breathing problems.
- Rinse mouth with water and spit out after each use to prevent oral thrush.
- Learn proper inhaler technique from your doctor or pharmacist.
Missed Dose Advice
If a dose is missed, take the next dose at the regularly scheduled time. Do not take a double dose to make up for a missed one.
Driving Precautions
Salomax B is generally not expected to affect the ability to drive or operate machinery. However, patients experiencing side effects like tremor or dizziness should exercise caution.
Lifestyle Advice
- Avoid known asthma/COPD triggers (e.g., smoke, allergens).
- Maintain a healthy lifestyle with regular exercise and a balanced diet.
- Quit smoking if you are a smoker.
