Pediatric
Dosage varies by age, weight, and indication. For chemotherapy-induced nausea and vomiting (CINV), typically 0.15 mg/kg orally every 4-8 hours for up to 3 doses or 4 mg for children >4 years. For drops, follow doctor's prescription, often 2-4 mg orally as a single dose or repeated.
Renal_impairment
No dosage adjustment is generally required for patients with renal impairment.
How to Take
Administer orally. Measure the prescribed dose accurately using the provided dropper or oral syringe. Can be taken with or without food.
Mechanism of Action
Ondansetron is a selective 5-hydroxytryptamine3 (5-HT3) receptor antagonist. Chemotherapeutic agents and radiation therapy can cause the release of serotonin (5-HT) from enterochromaffin cells in the small intestine, which then activates 5-HT3 receptors located on vagal afferent nerves, initiating the vomiting reflex. Ondansetron blocks these 5-HT3 receptors both peripherally (on vagal nerve terminals) and centrally (in the chemoreceptor trigger zone, CTZ) to prevent nausea and vomiting.

Onset

Excretion
Excreted in urine (about 5-10% as unchanged drug) and feces (metabolites).
Half life
Approximately 3-6 hours in children (varies by age).
Absorption
Rapidly and completely absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are achieved in about 1.5 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP1A2, CYP2D6, CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to Ondansetron or any component of the formulation.
- •Concomitant use with apomorphine due to risk of profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol; concomitant use should be avoided if possible. Increases risk of serotonin syndrome.
Apomorphine
Concomitant use is contraindicated due to reports of profound hypotension and loss of consciousness.
SSRIs/SNRIs
Increased risk of serotonin syndrome with other serotonergic agents.
Phenytoin, Carbamazepine, Rifampicin
May decrease plasma concentrations of ondansetron due to enzyme induction.
Storage
Store below 30°C in a dry place, away from light. Do not freeze.
Overdose
Symptoms of overdose include transient visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Ondansetron is excreted in human milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
