Overview of the medicine
Sapox 125 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections in children.
Uses & Indications
Dosage
Adults
Adults typically use tablet forms. If suspension is used, consult a physician for appropriate dosing based on weight and infection severity (e.g., 400 mg once daily).
Children
For children 6 months to 12 years: 8 mg/kg/day administered as a single dose or in two divided doses every 12 hours. Max 400 mg/day.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (creatinine clearance < 30 ml/min).
How to Take
Shake the bottle well before each use. Administer orally with a measuring spoon or cup. Can be taken with or without food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Onset
Typically, clinical improvement is observed within 24-48 hours.
Excretion
Primarily renal excretion (approximately 50% unchanged in urine), with some biliary excretion.
Half life
Plasma half-life is usually 3 to 4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the extent of absorption.

Cefixime is largely excreted unchanged; there is no evidence of significant metabolism.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase prothrombin time (INR), requiring closer monitoring.
Nifedipine
Increases peak plasma level and AUC of Cefixime.
Carbamazepine
May increase plasma concentrations of carbamazepine.
Oral Anticoagulants
Enhanced anticoagulant effect observed with some cephalosporins.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep out of reach of children. Store reconstituted suspension in a refrigerator (2-8°C); do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
