Description
Overview of the medicine
SB-Amlod is a long-acting calcium channel blocker used to treat high blood pressure (hypertension) and certain types of chest pain (angina). It works by relaxing blood vessels, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Initial 5 mg once daily; maximum 10 mg once daily. Dosage may be adjusted after 7-14 days.
Elderly
Initial 2.5 mg once daily. Use with caution.
Renal_impairment
No dosage adjustment required.
How to Take
Oral administration, with or without food. Can be taken at any time of day but ideally at the same time each day.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This reduces peripheral vascular resistance and lowers blood pressure. In angina, it dilates coronary arteries and arterioles, increasing myocardial oxygen delivery.

Onset
Onset of action for antihypertensive effect is 6-12 hours.
Excretion
Primarily via urine (60% inactive metabolites, 10% unchanged drug).
Half life
30-50 hours.
Absorption
Well absorbed orally, bioavailability 64-90%. Peak plasma concentrations within 6-12 hours.
Metabolism
Extensively metabolized in the liver to inactive metabolites (approx. 90%).
Side Effects
Contraindications
- •Hypersensitivity to amlodipine or dihydropyridine derivatives
- •Severe hypotension
- •Cardiogenic shock
- •Outflow tract obstruction of the left ventricle (e.g., high grade aortic stenosis)
- •Hemodynamically unstable heart failure after acute myocardial infarction
Drug Interactions
Sildenafil
Increased risk of hypotension. Administer sildenafil with caution.
Simvastatin
Limit simvastatin dose to 20 mg/day due to increased simvastatin exposure.
Digoxin, Warfarin
No significant interaction reported.
CYP3A4 inducers (e.g., rifampicin, St. John's Wort)
May decrease amlodipine exposure, reducing efficacy.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
May increase amlodipine exposure, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include excessive peripheral vasodilation with marked hypotension and possibly a reflex tachycardia. Management: supportive care, gastric lavage/activated charcoal, monitor cardiac and respiratory function, administer IV fluids and vasopressors if needed. Calcium gluconate may be beneficial.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts may pass into breast milk; caution advised during lactation.
