Description
Overview of the medicine
sb-ator 10 mg tablet contains Atorvastatin, which is used to lower high cholesterol and triglyceride levels in the blood. It helps prevent cardiovascular diseases like heart attack and stroke.
Uses & Indications
Dosage
Adults
Initial dose is typically 10-20 mg once daily. Dosage can be adjusted to 10-80 mg once daily based on LDL-C levels and therapeutic response. Max 80 mg/day.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
Renal impairment does not significantly affect atorvastatin plasma concentrations; therefore, no dosage adjustment is necessary.
How to Take
Take sb-ator 10 mg tablet orally once daily, with or without food, at any time of day. It is recommended to take it at the same time each day to help remember.
Mechanism of Action
Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme crucial for cholesterol synthesis in the liver. This leads to a reduction in hepatic cholesterol and an increase in LDL receptors on the cell surface, enhancing LDL uptake and catabolism.
Pharmacokinetics
Onset
Lipid-lowering effects typically observed within 2 weeks, with maximal effect usually achieved by 4 weeks of treatment.
Excretion
Primarily excreted in bile following hepatic and/or extra-hepatic metabolism. Less than 2% of the dose is recovered in urine.
Half life
The elimination half-life is approximately 14 hours; however, the half-life of HMG-CoA reductase inhibitory activity due to active metabolites extends to 20-30 hours.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached within 1-2 hours. Absolute bioavailability is approximately 12-14%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 3A4 (CYP3A4) to ortho- and para-hydroxylated derivatives and various beta-oxidation products, which are active.
Side Effects
Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Hypersensitivity to atorvastatin or any component of the formulation
- Co-administration with glecaprevir/pibrentasvir
Drug Interactions
Colchicine
Cases of myopathy, including rhabdomyolysis, have been reported with co-administration.
Cyclosporine
Increased atorvastatin levels, significantly increased risk of myopathy/rhabdomyolysis.
Niacin (lipid-lowering doses)
Increased risk of muscle side effects, including rhabdomyolysis.
Gemfibrozil and other fibrates
Increased risk of myopathy/rhabdomyolysis when co-administered with atorvastatin.
Strong CYP3A4 inhibitors (e.g., clarithromycin, itraconazole, protease inhibitors)
Significantly increased atorvastatin levels, increasing the risk of myopathy.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment or antidote for sb-ator overdose. In case of overdose, the patient should be treated symptomatically, and supportive measures instituted as required. Liver function tests and CK levels should be monitored.
Pregnancy & Lactation
sb-ator is contraindicated during pregnancy and lactation. Statins can cause fetal harm when administered to pregnant women. Women of childbearing potential should use effective contraception. It is unknown whether atorvastatin is excreted in human milk.

YesClinical Trials
Extensive clinical trials, including the ASCOT-LLA and MIRACL trials, have demonstrated the efficacy of atorvastatin in reducing LDL-C and preventing major cardiovascular events.
Lab Monitoring
- Liver function tests (LFTs) should be performed before initiation of therapy and as clinically indicated thereafter.
- Lipid panel (LDL-C, HDL-C, Total Cholesterol, Triglycerides) should be monitored regularly to assess efficacy.
- Creatine Kinase (CK) levels should be measured prior to initiation in patients with predisposing factors for myopathy, and if muscle symptoms occur.
Doctor Notes
- Emphasize the importance of lifestyle modifications (diet, exercise) alongside atorvastatin therapy.
- Monitor liver function tests and lipid profiles at baseline and periodically during treatment.
- Counsel patients on muscle-related symptoms (pain, weakness, dark urine) and the need for prompt reporting.
- Review potential drug interactions, especially with CYP3A4 inhibitors and other lipid-lowering agents.
Patient Guidelines
- Take sb-ator exactly as prescribed by your doctor, usually once daily.
- Do not stop taking the medicine suddenly without consulting your doctor, even if you feel well.
- Report any unexplained muscle pain, tenderness, or weakness, especially if accompanied by fever or dark urine, to your doctor immediately.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next scheduled dose (within 8-12 hours), skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
sb-ator is generally not expected to affect the ability to drive or operate machinery. However, some individuals may experience dizziness, so caution should be exercised until you know how you react to the medication.
Lifestyle Advice
- Adopt a low-cholesterol, low-saturated fat diet as recommended by your doctor or nutritionist.
- Engage in regular physical activity appropriate for your health condition.
- Avoid smoking and limit alcohol consumption to help manage cholesterol levels.
