Description
Overview of the medicine
SB-Ator 20 mg Tablet is an HMG-CoA reductase inhibitor (statin) used to lower cholesterol and triglyceride levels in the blood, thereby reducing the risk of heart disease and stroke.
Uses & Indications
Dosage
Adults
Initial dose is typically 10-20 mg once daily. The dose can be adjusted up to 80 mg once daily based on patient response and lipid goals. Administer at any time of day, with or without food.
Elderly
No specific dose adjustment is required based on age. However, use with caution due to potential increased susceptibility to adverse effects.
Renal_impairment
Renal impairment does not significantly affect plasma concentrations of atorvastatin. No dosage adjustment is necessary for patients with renal disease.
How to Take
Take SB-Ator 20 mg tablet orally, once daily, with or without food. It can be taken at any time of the day, but consistency is recommended.
Mechanism of Action
Atorvastatin selectively and competitively inhibits HMG-CoA reductase, the enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis, leading to a reduction in liver cholesterol synthesis and increased LDL receptor expression on hepatocyte surfaces, resulting in increased uptake and catabolism of LDL from the blood.
Pharmacokinetics
Onset
Significant lipid-lowering effects observed within 2 weeks, with maximal response usually achieved within 4 weeks.
Excretion
Primarily excreted in bile following hepatic and/or extrahepatic metabolism; less than 2% is recovered in urine.
Half life
Approximately 14 hours (for parent drug) and 20-30 hours (for active metabolites), primarily due to enterohepatic recirculation.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations achieved within 1-2 hours. Absolute bioavailability is approximately 12%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 3A4 (CYP3A4) to active ortho- and parahydroxylated metabolites and various beta-oxidation products.
Side Effects
Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases.
- Pregnancy and lactation.
- Hypersensitivity to Atorvastatin or any component of the formulation.
Drug Interactions
Warfarin
Potentiation of anticoagulant effect, requiring careful monitoring of INR.
Oral contraceptives
Increased AUC of norethindrone and ethinyl estradiol.
Cyclosporine, gemfibrozil, other fibrates, niacin
Increased risk of myopathy/rhabdomyolysis.
CYP3A4 inhibitors (e.g., itraconazole, ketoconazole, erythromycin, clarithromycin, HIV protease inhibitors)
Increased plasma concentrations of atorvastatin, potentially increasing risk of adverse effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for atorvastatin overdose. In the event of an overdose, the patient should be treated symptomatically, and supportive measures instituted as required. Due to extensive drug binding to plasma proteins, hemodialysis is not expected to significantly enhance atorvastatin clearance.
Pregnancy & Lactation
Atorvastatin is contraindicated during pregnancy and breastfeeding. Women of childbearing potential should use adequate contraception during treatment.

YesClinical Trials
Atorvastatin has been extensively studied in numerous clinical trials (e.g., ASCOT-LLA, MIRACL, TNT) demonstrating its efficacy in lowering LDL-C and reducing cardiovascular morbidity and mortality across various patient populations.
Lab Monitoring
- Liver function tests (LFTs) should be performed before the initiation of treatment and periodically thereafter.
- Creatine kinase (CK) levels should be measured if muscle symptoms (pain, tenderness, weakness) occur.
- Lipid panel (total cholesterol, LDL-C, HDL-C, triglycerides) should be monitored to assess treatment response.
Doctor Notes
- Emphasize the importance of lifestyle modifications (diet, exercise) alongside pharmacotherapy.
- Regularly monitor liver function tests (LFTs) and lipid profile.
- Educate patients on symptoms of myopathy and advise immediate reporting.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Inform your doctor immediately if you experience unexplained muscle pain, tenderness, or weakness, especially if accompanied by fever or dark urine.
