Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract, skin, ear, eye, and sexually transmitted infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days OR 500 mg as a single dose on Day 1, followed by 250 mg once daily on Days 2-5. For Chlamydia: 1g as a single oral dose.
Elderly
No dosage adjustment is necessary for elderly patients with normal renal and hepatic function. Caution is advised due to potential for QT prolongation.
Renal_impairment
No dosage adjustment is needed for mild to moderate renal impairment (creatinine clearance >10 mL/min). Exercise caution in severe impairment.
How to Take
Tablets and capsules can be taken with or without food. Oral suspension should be taken on an empty stomach (at least 1 hour before or 2 hours after a meal) for optimal absorption. Always complete the full prescribed course.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting bacterial protein synthesis. It is generally considered bacteriostatic but can be bactericidal at high concentrations against susceptible organisms.

Onset
Clinical effects typically within 24-48 hours.
Excretion
Primarily excreted unchanged in bile; minor renal excretion.
Half life
Terminal elimination half-life is 2-4 days (due to extensive tissue uptake and slow release).
Absorption
Rapidly absorbed after oral administration, bioavailability is approximately 37%. Peak plasma concentrations occur 2-3 hours after dosing.
Metabolism
Limited hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- Patients with a history of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased digoxin levels; monitor digoxin levels.
Antacids
Can decrease peak plasma levels of azithromycin; take azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Increased anticoagulant effect; monitor PT/INR closely.
Nelfinavir
Increased azithromycin concentration; no dose adjustment needed, but monitor for side effects.
QT-prolonging drugs
Additive risk of QT prolongation; avoid concomitant use if possible.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored at room temperature or refrigerated and used within 10 days.
Overdose
In case of overdose, gastric lavage and general supportive measures are indicated. Hemodialysis and peritoneal dialysis are not effective.

Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk; use with caution during lactation. Consult doctor.
