Description
Overview of the medicine
sb-gabalin 25 mg capsule contains Gabapentin, an anticonvulsant used to treat certain types of seizures and relieve neuropathic pain, such as postherpetic neuralgia.
Uses & Indications
Dosage
Adults
Starting dose usually 300 mg daily, titrated up to 1800-3600 mg/day in 3 divided doses. 25 mg is a very low dose, typically used for initial titration or highly sensitive patients. Follow physician's advice.
Elderly
Lower starting dose and slower titration due to potential age-related decrease in renal function. Monitor for adverse effects.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For CrCl 30-59 mL/min, total daily dose 400-1100 mg; for CrCl 15-29 mL/min, total daily dose 200-500 mg; for CrCl <15 mL/min, total daily dose 100-300 mg. Administer once daily for severe impairment.
How to Take
Take sb-gabalin 25 mg capsule orally, with or without food. Swallow the capsule whole with water. Do not crush, chew, or break the capsule.
Mechanism of Action
Gabapentin is structurally related to the neurotransmitter GABA but does not act on GABA receptors. Its exact mechanism is not fully understood, but it is believed to bind to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, which leads to a reduction in the release of excitatory neurotransmitters.
Pharmacokinetics
Onset
Therapeutic effects for pain may take several days to weeks; for epilepsy, effects are generally observed with titration.
Excretion
Excreted unchanged primarily by the kidneys.
Half life
Approximately 5 to 7 hours in adults with normal renal function.
Absorption
Oral absorption is dose-dependent and saturable; bioavailability decreases with increasing dose. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Not significantly metabolized in humans; it does not induce hepatic enzymes.
Side Effects
Contraindications
- •Hypersensitivity to gabapentin or any component of the formulation.
Drug Interactions
Opioids (e.g., Morphine, Hydrocodone)
Increased risk of respiratory depression, sedation, and potentially coma. Use with caution; monitor patients closely.
Antacids (containing aluminum and magnesium)
May decrease gabapentin bioavailability. Separate administration by at least 2 hours.
Central Nervous System (CNS) Depressants (e.g., Alcohol, Sedatives)
Additive CNS depressant effects leading to increased drowsiness, dizziness, and sedation.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, drowsiness, blurred vision, slurred speech, lethargy, and mild diarrhea. Management is supportive care. Hemodialysis can remove gabapentin from the plasma in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Gabapentin is excreted in human milk; caution should be exercised when administered to a nursing woman.
