Description
Overview of the medicine
Gabapentin is an anticonvulsant and neuropathic pain medication. It is used to treat partial seizures in epilepsy and neuropathic pain conditions like postherpetic neuralgia.
Uses & Indications
Dosage
Adults
Initial dose is typically 300 mg daily, titrated up to 1800-3600 mg daily in three divided doses for epilepsy. For neuropathic pain, initial dose 300 mg daily, titrated as needed. 50 mg capsules are usually for very sensitive patients or at the start of titration.
Elderly
Lower initial doses and slower titration are recommended due to potential decreased renal function. Renal function should be monitored.
Renal_impairment
Dose adjustment is required based on creatinine clearance. For severe impairment, dose frequency may be reduced.
How to Take
Take orally with or without food. Capsules should be swallowed whole with water. Do not chew, crush, or break the capsules.
Mechanism of Action
The exact mechanism of gabapentin is unknown, but it is thought to affect voltage-gated calcium channels, thereby reducing the release of excitatory neurotransmitters.

Contraindications

Drug Interactions
Antacids
Concurrent use of antacids containing aluminum and magnesium can reduce the bioavailability of gabapentin. Take gabapentin at least 2 hours after antacid administration.
Opioids (e.g., Morphine)
May increase gabapentin concentrations and enhance CNS depression, leading to increased sedation, dizziness, and respiratory depression. Monitor patients for signs of CNS depression.
Alcohol and other CNS depressants
May potentiate the sedative effects of gabapentin.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, drowsiness, lethargy, blurred vision, slurred speech, and mild diarrhea. Management is primarily supportive; hemodialysis can effectively remove gabapentin from plasma.
Pregnancy & Lactation
Pregnancy Category C. Gabapentin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; use with caution in nursing mothers.
