Description
Overview of the medicine
Losartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension), diabetic nephropathy, and heart failure.
Uses & Indications
Dosage
Adults
Initial dose is usually 50 mg once daily. For 25 mg: May be used as a starting dose in specific conditions or in patients who may be volume depleted. Maintenance dose: 25-100 mg once daily.
Elderly
No initial dosage adjustment is usually required, but a lower starting dose (e.g., 25 mg) may be considered in some elderly patients.
Renal_impairment
No initial dosage adjustment is required for mild to moderate renal impairment. A lower starting dose (e.g., 25 mg) may be considered in severe renal impairment.
How to Take
Take orally, with or without food. It can be taken at any time of the day, but it is recommended to take it at the same time each day to maintain consistent blood levels.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues. This prevents angiotensin II from causing vasoconstriction and aldosterone release, leading to reduced blood pressure.
Pharmacokinetics
Onset
Peak effect typically occurs within 6 hours. Full antihypertensive effect may take 3-6 weeks.
Excretion
Approximately 35% excreted in urine (4% unchanged losartan, 6% active metabolite) and 60% in feces (43% active metabolite).
Half life
Losartan: Approximately 2 hours. Active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed from the gastrointestinal tract, undergoes significant first-pass metabolism. Bioavailability is about 33%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9 and CYP3A4, to an active carboxylic acid metabolite (E-3174) and inactive metabolites.
Side Effects
Contraindications
- Pregnancy (second and third trimesters)
- Hypersensitivity to Losartan or any component of the formulation
- Co-administration with aliskiren in patients with diabetes mellitus or renal impairment
Drug Interactions
Lithium
May increase serum lithium levels, leading to lithium toxicity.
Aliskiren
Concomitant use in diabetic patients or those with renal impairment is contraindicated due to increased risk of renal impairment, hyperkalemia, and hypotension.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics & Potassium supplements
May increase the risk of hyperkalemia.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) could occur. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Category D in pregnancy (second and third trimesters) due to fetal toxicity. Contraindicated. Not recommended during breastfeeding as it is unknown if Losartan is excreted in human milk.
