Description
Overview of the medicine
Omeprazole is a proton pump inhibitor used to treat conditions where there is too much acid in the stomach, such as gastroesophageal reflux disease (GERD), ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. Duodenal Ulcer: 20 mg once daily for 2-4 weeks. Gastric Ulcer: 20 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted based on response. H. pylori eradication: 20 mg twice daily with appropriate antibiotics for 7-14 days.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is required for patients with renal impairment.
Hepatic_impairment
In patients with severe hepatic impairment, the dose should not exceed 20 mg daily.
How to Take
Take orally, preferably in the morning, at least 30 minutes before a meal. Swallow the capsule whole with water. Do not chew, crush, or open the capsule, as this may affect its enteric coating and drug release.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells. This action effectively blocks the final step in acid production, leading to a sustained reduction of gastric acid secretion.
Pharmacokinetics
Onset
Acid inhibitory effect starts within 1-2 hours. Full therapeutic effect within 2-4 days of continuous dosing.
Excretion
Approximately 77% of the dose is excreted in the urine as metabolites, and the remainder is excreted in the feces.
Half life
Plasma elimination half-life is approximately 0.5-1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed from the small intestine after oral administration. Bioavailability is approximately 30-40% after a single dose, increasing to about 60% with repeated administration due to saturation of first-pass metabolism.
Metabolism
Extensively metabolized in the liver primarily by the cytochrome P450 enzyme system, mainly CYP2C19 and CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- •Co-administration with nelfinavir (an HIV protease inhibitor).
Drug Interactions
Digoxin
Increased systemic exposure to digoxin, requiring monitoring of digoxin levels.
Diazepam
Decreased elimination of diazepam, leading to prolonged half-life and increased effects.
Warfarin
Potentiation of anticoagulant effect, leading to increased INR and risk of bleeding.
Tacrolimus
Increased plasma concentrations of tacrolimus, requiring monitoring and dose adjustment.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19, which is essential for clopidogrel's activation.
Methotrexate
Increased and prolonged elevation of methotrexate and/or its metabolite serum levels, potentially leading to methotrexate toxicity.
HIV Antivirals (e.g., Atazanavir, Nelfinavir)
Significantly reduced plasma concentrations of these antivirals, potentially leading to loss of therapeutic effect and development of resistance.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and self-limiting, including nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. There is no specific antidote. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Omeprazole should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
