Description
Overview of the medicine
A topical cream containing Permethrin, used for the treatment of scabies and lice infestations.
Uses & Indications
Dosage
Adults
For scabies, apply Scabicid 5% cream to the entire body from the neck down. Leave on for 8-14 hours, then wash off thoroughly. A single application is usually sufficient, but a repeat application may be needed after 7-10 days if live mites or new lesions appear.
Elderly
Same as adult dosage.
Renal_impairment
No specific dosage adjustment is necessary due to minimal systemic absorption.
How to Take
For topical use only. Apply a thin layer of cream to clean, dry skin. Rub in gently until absorbed. Avoid contact with eyes, mouth, nose, and open wounds.
Mechanism of Action
Permethrin acts on the nerve cell membrane of parasites to disrupt the sodium channel current, which regulates polarization of the membrane. This leads to delayed repolarization and subsequent paralysis and death of the parasite.
Pharmacokinetics
Onset
Mite killing effect within hours of application.
Excretion
Primarily excreted in the urine as inactive metabolites.
Half life
Approximately 1-2 hours after topical application.
Absorption
Minimal systemic absorption (less than 2%) through intact skin following topical application.
Metabolism
Rapidly metabolized by ester hydrolysis to inactive metabolites in the liver and skin.
Side Effects
Contraindications
- •Hypersensitivity to permethrin, other pyrethroids, pyrethrins, or any component of the cream.
- •Infants under 2 months of age (for 5% cream).
Drug Interactions
0
No significant drug interactions have been reported with topical permethrin.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Do not freeze.
Overdose
Over-application may cause increased local irritation such as redness, swelling, and burning. Systemic overdose is unlikely due to poor dermal absorption. Management involves washing off the cream and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category B. Permethrin is generally considered safe for use during pregnancy when clinically indicated. It is minimally absorbed systemically. Consult a doctor. Limited data suggests minimal excretion into breast milk, but caution is advised during lactation.
