Description
Overview of the medicine
Secoclav 250 mg Tablet is a combination antibiotic containing Amoxicillin and Clavulanic Acid. It is used to treat a wide range of bacterial infections, including those resistant to amoxicillin alone due to beta-lactamase production.
Uses & Indications
Dosage
Adults
One Secoclav 250 mg tablet three times a day for 7-10 days, or as directed by a physician. Dosage may vary based on infection severity and site.
Elderly
No specific dose adjustment is generally required unless there is evidence of renal impairment.
Renal_impairment
Dose adjustment is necessary for patients with impaired renal function; specific recommendations depend on the creatinine clearance.
How to Take
Take the tablet orally, preferably at the start of a meal to minimize potential gastrointestinal intolerance. Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis, leading to bacterial death. Clavulanic acid is a beta-lactamase inhibitor that protects amoxicillin from degradation by bacterial beta-lactamase enzymes, thereby extending amoxicillin's antibacterial spectrum.
Pharmacokinetics
Onset
Peak plasma concentrations reached approximately 1-2 hours after oral administration.
Excretion
Mainly excreted unchanged in the urine (amoxicillin) and as metabolites (clavulanic acid) via renal excretion.
Half life
Amoxicillin: approximately 1 to 1.5 hours; Clavulanic Acid: approximately 1 hour.
Absorption
Well absorbed orally; absorption is optimized when taken at the start of a meal.
Metabolism
Amoxicillin undergoes limited hepatic metabolism. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- History of hypersensitivity to penicillin or any other beta-lactam antibiotics
- History of amoxicillin/clavulanic acid-associated jaundice or hepatic dysfunction
- Severe renal impairment (e.g., anuria, hemodialysis) without appropriate dose adjustment
Drug Interactions
Probenecid
Reduces renal tubular secretion of amoxicillin, leading to increased and prolonged blood levels of amoxicillin. Not recommended for concurrent use.
Allopurinol
Concomitant use may increase the incidence of skin rash.
Methotrexate
May increase methotrexate toxicity due to reduced renal excretion.
Oral contraceptives
May reduce the efficacy of oral contraceptives; advise patients to use additional non-hormonal contraception.
Warfarin and other oral anticoagulants
May prolong prothrombin time (INR); increased risk of bleeding. Close monitoring of INR is recommended.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. Management is symptomatic and supportive. Gastric lavage may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed after careful consideration of benefits vs. risks. Amoxicillin and clavulanic acid are excreted in breast milk; use with caution in lactating mothers.

Lab Monitoring
- Liver function tests (LFTs) should be monitored periodically during prolonged therapy or in patients with a history of hepatic dysfunction.
- Renal function tests may be considered in patients with pre-existing renal impairment.
Doctor Notes
- Emphasize the importance of completing the full course of treatment to prevent antibiotic resistance.
- Advise patients to report any signs of hypersensitivity reactions or hepatic dysfunction promptly.
- Consider renal function when prescribing, especially in elderly patients.
