Description
Overview of the medicine
Secoclav 500 mg tablet is a broad-spectrum antibiotic combination of amoxicillin and clavulanic acid, used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
One 500 mg/125 mg tablet every 8 hours, or as directed by physician.
Elderly
Dosage adjustment not typically required unless there is renal impairment.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance; e.g., CrCl 10-30 mL/min: 500 mg/125 mg every 12 hours; CrCl <10 mL/min: 500 mg/125 mg every 24 hours.
How to Take
Take orally at the start of a meal to minimize potential gastrointestinal intolerance.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis. Clavulanic acid is a beta-lactamase inhibitor that prevents bacterial enzymes from inactivating amoxicillin, thus extending its spectrum of activity.
Pharmacokinetics
Onset
Peak plasma concentrations are usually achieved within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine (amoxicillin) and as metabolites (clavulanic acid).
Half life
Amoxicillin: approximately 1-1.3 hours; Clavulanic acid: approximately 1 hour.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, optimized when taken at the start of a meal.
Metabolism
Amoxicillin is partially metabolized; Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to penicillin or other beta-lactam antibiotics.
- History of amoxicillin/clavulanate-associated jaundice or hepatic dysfunction.
- Infectious mononucleosis (increased risk of rash).
Drug Interactions
Warfarin
May prolong prothrombin time; monitor INR closely.
Probenecid
Increases and prolongs amoxicillin blood levels by decreasing renal tubular secretion.
Methotrexate
Increased methotrexate toxicity due to reduced renal excretion.
Oral contraceptives
May reduce efficacy of oral contraceptives (advise additional contraceptive method).
Storage
Store in a cool, dry place below 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms include gastrointestinal discomfort, fluid and electrolyte imbalance. Treatment is symptomatic and supportive. Hemodialysis may be used to remove amoxicillin/clavulanate from circulation.
Pregnancy & Lactation
Pregnancy category B. Use with caution during pregnancy if clearly needed. Excreted in breast milk, use with caution during lactation.

- Renal function should be monitored in patients with renal impairment.
Doctor Notes
- Advise patients to complete the full course to prevent resistance.
- Caution in patients with hepatic impairment or history of jaundice.
- Monitor for signs of allergic reactions, including rash, itching, or swelling.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Take with food to reduce stomach upset.
- Report any severe diarrhea, rash, or yellowing of the skin/eyes immediately.
Missed Dose Advice
Take the missed dose as soon as you remember, unless it is almost time for your next dose. Do not double the dose.
