Description
Overview of the medicine
Seemaxyl 125 mg Suspension is an oral third-generation cephalosporin antibiotic containing Cefixime, used to treat a wide range of bacterial infections in children.
Uses & Indications
Dosage
Adults
Adults usually take 200-400 mg daily. For pediatric suspension, adult use is uncommon unless specifically prescribed.
Elderly
No specific dosage adjustment is required for elderly patients unless there is severe renal impairment.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance less than 60 mL/min. For CrCl 21-60 mL/min, 75% of standard dose; for CrCl < 20 mL/min, 50% of standard dose.
Children_6_months_to_12_years
8 mg/kg/day administered as a single dose or in two divided doses (4 mg/kg every 12 hours). The 125 mg/5 mL suspension provides 25 mg/mL.
How to Take
For oral administration. Shake the bottle well before each use. Can be taken with or without food. Complete the full course of treatment as prescribed by the doctor.
Mechanism of Action
Cefixime exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%) within 24 hours.
Half life
Plasma elimination half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly alter its extent.
Metabolism
Not extensively metabolized in the liver; some inactive metabolites have been detected.
Side Effects
Contraindications
- Hypersensitivity to cefixime or other cephalosporins
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics
Drug Interactions
Warfarin
May increase prothrombin time (PT) and International Normalized Ratio (INR), leading to an increased risk of bleeding. Close monitoring of coagulation parameters is advised.
Nifedipine
May increase cefixime Cmax and AUC.
Carbamazepine
Concomitant use may increase carbamazepine plasma levels.
Storage
Store dry powder below 30°C, protected from light and moisture. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days. Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Treatment should be supportive. Hemodialysis or peritoneal dialysis does not effectively remove cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Cefixime is excreted in small amounts in breast milk; caution is advised during breastfeeding.
