Overview of the medicine
Sefrate (Cefixime) is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those affecting the respiratory tract, urinary tract, and ear, nose, and throat.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing more than 50 kg: 200 mg to 400 mg once daily or in two divided doses for 7-14 days depending on the infection.
Elderly
No specific dosage adjustment is required for the elderly unless there is severe renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, total daily dose should not exceed 300 mg. For creatinine clearance < 20 mL/min, total daily dose should not exceed 200 mg.
How to Take
Sefrate can be taken with or without food. For suspension, reconstitute the powder with boiled and cooled water as directed on the label.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours post-oral administration.
Excretion
Approximately 50% of an absorbed dose is excreted unchanged in the urine over 24 hours. The remainder is excreted by non-renal mechanisms, likely biliary excretion.
Half life
Average plasma half-life of 3-4 hours, which may be prolonged in patients with renal impairment.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect overall absorption.
Metabolism
Not extensively metabolized in the body; primarily excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- •History of severe allergic reactions to penicillin (cross-reactivity may occur).
Drug Interactions
Probenecid
May increase Cefixime plasma levels by reducing renal tubular secretion.
Carbamazepine
Concurrent use with Cefixime has been reported to increase Carbamazepine levels.
Oral anticoagulants (e.g., Warfarin)
May increase prothrombin time and bleeding risk. Monitor INR carefully.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Do not freeze. Keep the bottle tightly closed.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Hemodialysis or peritoneal dialysis are not significantly effective in removing Cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed after consulting with a doctor. Cefixime is excreted in small amounts in breast milk; use with caution during lactation and monitor the infant for gastrointestinal disturbances.
