Overview of the medicine
Sefril DS (Cefixime) is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, urinary tract infections, and gonorrhoea.
Uses & Indications
Dosage
Adults
For adults and children over 12 years or weighing more than 50 kg, the usual dose is 400 mg once daily or 200 mg twice daily. Treatment duration usually 7-14 days depending on infection.
Elderly
No dosage adjustment is generally required for elderly patients unless renal function is significantly impaired.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (creatinine clearance < 30 mL/min). A dose of 200 mg once daily is recommended.
How to Take
Sefril DS should be taken orally, with or without food. To enhance absorption and minimize gastrointestinal upset, it can be taken with food.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which are responsible for the final stage of peptidoglycan synthesis. This disruption leads to an unstable cell wall, eventually causing bacterial cell lysis and death.

Onset
Clinical effects typically observed within a few hours of administration.
Excretion
Excreted primarily in the urine (about 50%) and bile/feces (about 10%) as unchanged drug.
Half life
The plasma half-life averages 3 to 4 hours, but may be prolonged in patients with impaired renal function.
Absorption
Approximately 40-50% of an oral dose of cefixime is absorbed from the gastrointestinal tract, with or without food.
Metabolism
Cefixime is not extensively metabolized in the body; about 50% of the absorbed dose is excreted unchanged in the urine.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or other cephalosporin antibiotics.
- History of hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Warfarin
Cefixime may enhance the anticoagulant effect of warfarin, leading to increased prothrombin time and INR.
Carbamazepine
Concomitant administration of cefixime with carbamazepine may result in increased carbamazepine plasma levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human milk; use with caution in nursing mothers.
