Description
Overview of the medicine
Selex 125 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including those of the urinary tract, respiratory tract, ears, nose, throat, and skin.
Uses & Indications
Dosage
Adults
Typically 200 mg twice daily or 400 mg once daily. For children (6 months to 12 years), 8 mg/kg/day once daily or in two divided doses.
Elderly
Similar to adults, but dosage adjustment may be necessary in elderly patients with significant renal impairment.
Renal_impairment
Dose adjustment required. For creatinine clearance 21-60 mL/min, reduce to 75% of standard dose. For creatinine clearance <20 mL/min, reduce to 50% of standard dose.
How to Take
Selex suspension is for oral use. It can be taken with or without food. Shake the bottle well before each use. Use the provided measuring device to ensure accurate dosing. Complete the full course of treatment as prescribed by your doctor, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which inhibits the third and final stage of bacterial cell wall synthesis. This leads to the lysis and death of the bacterial cell.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Primarily excreted unchanged in the urine (50-60%) and to a lesser extent in bile and feces.
Half life
About 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the total amount absorbed.
Metabolism
Not extensively metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime, other cephalosporin antibiotics, or any component of the formulation.
- History of severe hypersensitivity reaction to penicillin or other beta-lactam antibiotics (due to potential cross-reactivity).
Drug Interactions
Probenecid
Increases plasma concentration and prolongs the half-life of cefixime by decreasing renal excretion.
Carbamazepine
Concomitant use with cefixime has been reported to increase carbamazepine plasma concentrations.
Oral contraceptives
Some cephalosporins may reduce the effectiveness of oral contraceptives, although this is less documented for cefixime. Advise backup contraception.
Warfarin and other anticoagulants
May increase prothrombin time, potentially leading to increased bleeding risk. Monitor INR/PT carefully.
Storage
Store the dry powder below 30°C, protected from light and moisture. Keep out of reach of children. After reconstitution, store the suspension in a refrigerator (2-8°C) and do not freeze. Discard any unused portion after the specified period.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B: Animal reproduction studies have shown no risk, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Lactation: Cefixime is excreted in breast milk in small amounts. Use with caution in nursing mothers.
