Description
Overview of the medicine
Sephar 125 mg Suspension is an antibiotic medication containing Cephalexin, a first-generation cephalosporin. It is used to treat a wide range of bacterial infections including respiratory tract infections, ear infections, skin and soft tissue infections, and urinary tract infections.
Uses & Indications
Dosage
Adults
Not typically prescribed for adults in this strength. Adult dosage usually ranges from 250 mg to 1 g every 6 hours, depending on the infection.
Elderly
Dosage adjustment may be necessary for elderly patients with renal impairment. Consult a physician.
Renal_impairment
For patients with creatinine clearance < 40 mL/min, dosage intervals should be extended. e.g., 250 mg every 8-12 hours for mild impairment.
How to Take
Oral administration. Shake the bottle well before each use. Can be taken with or without food. Complete the full course of medication as prescribed, even if symptoms improve.
Mechanism of Action
Cephalexin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Approximately 1 hour (oral)
Excretion
Mainly via the kidneys by glomerular filtration and tubular secretion. Approximately 70-90% is excreted unchanged in urine within 8 hours.
Half life
0.5 to 1.2 hours (normal renal function)
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Food may delay absorption but does not significantly reduce its extent.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cephalexin or other cephalosporins
- History of severe hypersensitivity reactions to penicillin (e.g., anaphylaxis)
Drug Interactions
Metformin
Co-administration with cephalexin may increase metformin plasma concentrations.
Probenecid
May increase cephalexin plasma concentrations and prolong its half-life by decreasing renal tubular secretion.
Live Typhoid Vaccine
Antibiotics may reduce the therapeutic effect of live typhoid vaccine. Administer at least 3 days apart.
Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect of warfarin and other oral anticoagulants, requiring INR monitoring.
Storage
Store dry powder below 30°C. Keep away from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and discard any unused portion after 14 days, or store at room temperature (below 30°C) and discard after 7 days, as per manufacturer's instructions.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, diarrhea, and hematuria. Management is supportive; gastric lavage may be indicated, and hemodialysis may be helpful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Cephalexin is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother. Generally considered safe, but use only if clearly needed and benefits outweigh risks.
